疑诊老年肺癌患者p53基因突变检测及意义

目的　探讨纤维支气管镜标本的p5 3基因突变检测对老年肺癌早期诊断的价值。方法　应用聚合酶链反应—单链构象多肽性分析 (PCR—SSCP)技术 ,对 5 4例临床高度疑诊肺癌的老年患者纤维支气管镜标本进行p5 3基因 5～ 8号外显子突变分析 ,分析结果与病理学对照或随访以证实阳性的可信度。结果　 19例确诊老年肺癌患者中 ,有 10例 p5 3基因发生突变 ,占 5 3%。在 35例疑诊老年肺癌但病理学阴性的患者中发生p5 3基因突变的 11例 ,无p5 3基因突变的 2 4例。经过 2 8个月的随访 ,p5 3基因突变组有 5例证实为肺癌 ,占 4 6 % ;而无p5 3基因突变组只有 2例证实为肺癌 ,占 9%。两组比较 ,差异具有显著性 (P <0 0 1)。结论　通过对患者纤维支气管镜标本的 p5 3基因突变分析 ,作为早期诊断老年肺癌的一种方法是可行的

Detection of p53 gene mutation and its significance in early diagnosis of suspicious senile lung cancer

Objective To discuss the application value of p53 gene mutation in early diagnosis of senile lung cancer.Methods Fifty four specimens of suspicious lung cancer patients were analyzed for their p53 gene mutation using PCR-SSCP.The results were compared with the pathological studies of the specimens.Follow up was carried out to confirm the credibility of the positive results. Results In the 19 old lung cancer patients,10 cases had p53 gene mutation,with a percentage 53%.Among the 35 suspicious lung cancer patients who were negative to pathological examination,11 had p53 gene mutation,and 24 had not.After a 28 month follow up,5 were diagnosed as lung cancer in p53 gene mutation group,with a rate 46%;only 2 were diagnosed as lung cancer in non p53 gene mutation group,accounting for 9%.There was a significant difference between the two groups.Conclusion By analyzing p53 gene mutation of patients′ bronchoscopic specimen,it is feasible to early diagnose the senile lung cancer.
