MRI对前列腺癌与前列腺肉瘤的鉴别诊断临床研究

目的探究磁共振成像(MRI)鉴别诊断前列腺癌(PCa)和前列腺肉瘤(PS)的临床价值。方法回顾性分析128例前列腺恶性肿瘤患者临床资料,以穿刺活检术病理学检测结果为"金标准",比较各序列鉴别诊断前列腺恶性肿瘤的准确性及影像特征差异。结果128例前列腺恶性肿瘤患者中,经由穿刺活检术病理学检出PCa 115例(89.84%)与PS 13例(10.16%)。MRI平扫、弥散加权成像(DWI)、动态对比增强(DCE)序列鉴别诊断前列腺恶性肿瘤准确率分别为87.50%、90.62%、92.97%,各序列间比较无统计学意义(P>0.05),且均与"金标准"鉴别诊断一致性尚可(0.7>Kappa≥0.4)。结论MRI鉴别诊断PCa与PS准确率均较高,能通过不同序列获得丰富诊断信息,有较高临床应用价值。

Clinical Study of Differential Diagnosis Between Prostate Cancer and Prostate Sarcoma by MRI

Objective To investigate the clinical value of magnetic resonance imaging(MRI)in the differential diagnosis of prostate cancer(PCa)and prostate sarcoma(PS).Methods The clinical data of 128 patients with malignant prostate tumors were retrospectively analyzed.The pathological examination results of the biopsy were taken as the gold standard to compare the accuracy and image features of each sequence in the differential diagnosis of malignant prostate tumors.Results Among 128 patients with malignant prostate tumors,115 cases(89.84%)of PCa and 13 cases(10.16%)of PS were detected by biopsy pathology.The accuracy rates of MRI plain scan,diffusion-weighted imaging(DWI),and dynamic contrast enhancement(DCE)sequences were 87.50%,90.62%,and 92.97%,respectively in the differential diagnosis of prostate malignant tumors(P>0.05),and all of them were consistent with the gold standa rd differential diagnosis(0.7>Ka ppa≥0.4).Conclusion The accu racy rate of MRI is high in the differential diagnosis of PCa and PS,and it can obtain rich diagnostic information through different sequences,and it has high clinical application value.
