婴儿微青春期维生素D水平与性腺功能的关系

摘    要:
目的探讨健康婴儿微青春期维生素D水平与性腺激素的关系。方法共100例婴儿纳入研究,其中男婴54例,女婴46例,在出生后30 d^45 d测定维生素D、LH、FSH、TT、E2、AMH和inhibin B水平。将所有婴儿分为维生素D缺乏(<10 ng/ml)、维生素D不足(10 ng/ml^20 ng/ml)和维生素D充足(>20 ng/ml)3组。分析维生素D水平与生殖激素的相关性。结果维生素缺乏组TT水平高于维生素充足组,差异有统计学意义[均值(0.51±0.31)ng/ml VS(0.27±0.22)ng/ml,P<0.05];而维生素D缺乏组的inhibin B则低于维生素充足组[均值:(21.1±15.62)pg/ml VS(53.36±39.36)pg/ml,P<0.05]。结论随机对照试验显示维生素D在微青春期对性腺激素的重要性。维生素D对微青春期女婴的TT和inhibin B有一定的作用,维生素D可能在生命早期对性腺功能有影响。

Abstract:
Objective To investigate the relationship between vitamin D levels and gonadal hormones in healthy infants.Methods A total of 100(54 boys and 46 girls)healthy appropriate-for-gestational-age neonates were included.vitamin D,LH,FSH,total testosterone,oestradiol,AMH and inhibin B levels were measured at postnatal 30 d‐45 d.All infants were divided into three groups including vitamin D deficiency(<10 ng/ml),vitamin D insufficiency(10 ng/ml-20 ng/ml)and vitamin D sufficiency(>20 ng/ml).Correlations between vitamin D status and reproductive hormones were analyzed.Results Total testosterone level was higher(mean:0.51 ng/ml±0.31 ng/ml VS 0.27 ng/ml±0.22 ng/ml,P=0.0197)and inhibin B was lower in vitamin D deficient than sufficient girls(mean:21.1 pg/ml±15.62 pg/ml VS 53.36 pg/ml±39.36 pg/ml,P=0.032).Conclusion Randomized controlled trials showed the importance of vitamin D to gonadal hormones during mini-puberty.A modest effect of vitamin D was identified on total testosterone and inhibin B in girls at mini-puberty and vitamin D may have an effect on gonadal function during early life.
