病毒性心肌炎患儿心肌酶、肌钙蛋白I和心电图检测及意义

目的探讨病毒性心肌炎患儿的心肌酶谱、肌钙蛋白(cTnI)、超敏C-反应蛋白(hs-CRP)和心电图的变化特征,为临床诊治提供理论依据。方法选取2016年6月-2018年7月在本院诊断为病毒性心肌炎的儿童84例(年龄3岁~8岁),选取同期健康体检的患儿30例作为对照组,检查比较二组患儿的血清心肌酶谱、cTnI、hs-CRP和心电图的差异。结果病毒性心肌炎患儿的LDH、CK-MB和AST均明显高于对照组。病毒性心肌炎患儿的cTnI和hs-CRP均明显高于对照组。病毒性心肌炎患儿cTnI阳性的其ECG异常率显著高于cTnI阴性患儿。结论病毒性心肌炎患儿进行心肌酶谱、cTnI、hs-CRP和心电图检查有助于早期发现心肌损伤,达到早期诊断早期治疗的目的。

Detection and significance of myocardial enzyme,troponin and electrocardiogram in children with viral myocarditis

Objective To investigate the changes of myocardial enzymes,troponin,hypersensitive CRP and electrocardiogram in children with viral myocarditis,and to provide theoretical basis for clinical diagnosis and treatment.Methods From June2016 to July 2018,84 children(aged 3 to 8 years old)with viral myocarditis were enrolled in our hospital.Thirty children with healthy physical examination were selected as the control group.Differences in serum myocardial zymogram,troponin,high-sensitivity CRP,and electrocardiogram were discussed.Results LDH,CK-MB and AST of children with viral myocarditis were significantly higher than those of the control group.Troponin I and hypersensitive CRP in children with viral myocarditis were significantly higher than those in the control group.The troponin T-positive ECG abnormal rate was significantly higher in children with viral myocarditis than in troponin I-negative children.Conclusion Myocardial zymogram,troponin I,hypersensitive CRP and electrocardiogram in children with viral myocarditis can help the early detection of myocardial injury and achieve early diagnosis and early treatment.
