口服米索前列醇预防产后出血的疗效观察

目的：观察口服米索前列醇预防产后出血的效果。方法：将正常的单胎头位足月妊娠阴道分娩204例,随机分为研究组（122例）和对照组（82例）,研究组在第二产程末胎头娩出前口服米索前列醇400μg,对照组在胎儿娩出后立刻肌肉注射缩宫素10 U。结果：与对照组比较,研究组第三产程平均时间缩短5.32 min（P〈0.01）;产后2 h内平均出血量减少118.9 mL,差异有统计学意义（P〈0.01）。结论：口服米索前列醇促进子宫收缩作用强于缩宫素,能减少产后出血量,且给药方法简便,值得临床推广应用。

Effects of oral misoprostol on postpartum hemorrhage

Objective:To investigate the effects of oral misoprostol on postpartum hemorrhage.Methods: 204 vaginal delivery cases with full-term pregnancy of singleton infant in cephalic presentation were divided into study group(122 cases) and control group(82 cases).The study group was given 400 μg oral misoprostol before head expulsion and after the second birth process.The control group was given 10 U intramuscular oxytocin at the expulsion of fetus.Results: For study group,the average time of the third delivery process was 5.32 min shorter than that of the control group(P<0.01),and the average hemorrhage volume was 118.9 mL less with significant differences(P<0.01).Conclusion: Oral misoprostol pills showed stronger uterine contraction effects than intramuscular oxytocin;and could effectively reduce postpartum hemorrhage volume.It worths application in large scale.
