新生儿腹股沟嵌疝的护理

目的 总结新生儿腹股沟嵌疝围手术期的护理要点,提高新生儿腹股沟嵌疝手术治愈率,降低术后并发症的发生.方法 分析我院1999～2006年收治的34例新生儿腹股沟嵌疝患儿的护理记录及临床资料,总结新生儿腹股沟嵌疝围手术期护理要点.结果 34例新生儿腹股沟嵌疝患儿,2例肌注镇静剂后体检时自行复位,Ⅱ期手术治愈.急诊手术32例,8例发生肠管坏死而行了肠切除肠吻合术.术前合并新生儿肺炎7例,新生儿硬肿症1例.经积极抗感染、营养支持、保暖等治疗,30例均在术后7～8d康复出院,2例因术后合并并发症放弃治疗后死亡.结论 新生儿腹股沟嵌疝是小儿外科的常见急症,早诊断、早治疗及正确的围手术期护理,是新生儿腹股沟嵌疝治疗的关键.

Care of neonatal inguinal incarcerated hernia

Objective To summarize the perioperative care of neonatal inguinal incarcerated hernia and to improve the surgery cure rate, reduce the incidence of postoperative complications. Methods Thirty-four admitted neonatal patients with inguinal incarcerated hernia containing the records and clinical care were analysised in our hospital within 1999-2006, and newborns inguinal incarcerated hernia perioperative nursing was summed up. Results Of 34 cases of newborn children with inguinal incarcerated hernia, 2 cases with tranquilizers injection were observed reduction after a medical examination and cured with 2nd operation. Thirty-two cases under emergency surgery, eight cases occurred intestinal necrosis and fixed with a bowel resection anastomosis. Preoperative combined neonatal pneumonia occurred in 7 cases, 1 neonatal scleredema. With aggressive anti-infection, nutrition support, and warming,30 cases were discharged on 7-8d after operation and 2 cases with complications were dead after giving up treatment. Conclusion Neonatal inguinal incarcerated hernia surgery is a common pediatric emergency. Early diagnosis and proper perioperative care are the key to the treatment of neonatal inguinal incarcerated hernia.
