Factors associated with commercial sexual behavior among men who have sex with men in Chongqing,China

Objective To investigate the status and factors associated with commercial sexual behavior among men who have sex with men(MSM)in Chongqing.Methods A convenience sampling method was used to recruit MSM in Chongqing from 2019 to 2020.Questionnaire-based interviews were conducted on a one-on-one basis.Data were collected,including social-demographic information,sexual characters and drug abuse.Blood samples(5 ml)were taken and tested for treponema pallidum and HIV antibodies.Factors associated with commercial sexual behavior were analyzed by binary multivariate unconditional logistic regression.Results Among the 1151 MSM,105(9.1%)reported they were involved in commercial sexual behavior.The HIV prevalence rate was 16.8%(193/1151).The prevalence rate among those ever involved in commercial sexual behavior was 23.8%(25/105)and the rate among MSM with no experience in commercial sexual was 16.1%(168/1046).Compared with the MSM with no experience in commercial sexual,those ever involved in commercial sexual behavior had a significantly higher prevalence rate of HIV(with values of 4.11,P=0.04).The results of the multivariate logistic regression analysis indicated that the education level of college or above(OR=1.52,95%CI:1.10-2.60),residence registration out of Chongqing(OR=1.90,95%CI:1.21-2.98),bisexual behavior(OR=2.03,95%CI:1.24-3.30),group sex(OR=1.71,95%CI:1.17-2.90),and drug abuse(OR=5.68,95%CI:3.51-9.17)were more likely to have commercial sexual behavior,compared with those who had high school education level and below,Chongqing local residence registration,homosexual behavior,non-group sex,and non-drug abuse.Compared with the MSM who drank regularly,the MSM occasionally/never drank were less likely to have commercial sexual behavior(OR=0.50,95%CI:0.30-0.86/OR=0.46,95%CI:0.25-0.85).Conclusion Education level,registered residence,history of alcohol,sexual orientation,history of group sex and drug abuse are associated with commercial sexual behavior.We should consider the factors while developing syphilis/HIV prevention program for MSM.
