外固定支架治疗严重开放性骨折针道感染的相关因素研究

目的分析应用外固定支架治疗严重开放性骨折针道感染的相关因素.方法对120例应用外固定支架治疗严重开放性骨折的针道分泌物或渗出物进行细菌培养,同时拟对11个感染的相关因素进行临床分析和单因素、多因素非条件logistic回归分析.结果发生针道感染11例,感染率9.0%.针道感染主要危险因素是固定时间,伤情,合用激素与否,和有无伴有糖尿病.结论针道感染的高危因素为固定时间,伤情,合用激素与否,和有无伴有糖尿病.因而,对有高危因素存在的支架固定治疗者固定针的针道要监测各项感染指标,避免针道感染的发生.

Risk factors for nail-tract infection resulted from treatment of severe open fractures with external fixation apparatus

To analyze the risk factors for the nail-tract infection resulted from the treatment of severe open fracture with external fixation apparatus. We documented prospective study of 120 patients suffered from severe open fractures after trauma treated with external fixation apparatus between 2001 and 2003. Bacterial cultures of the substance collected from about the fixed-nail or excreted from the nail-tract were performed by usual rute. 11 related factors were observed and analyzed by single and multiple logistic regression. 11 of 120 cases happened unfortunately to develop nail-related infection. The infected rate was 9.0%. According to the regressive analysis, the main factors of nail-related infection were the nail placing time, severity of trauma, hormone applying and associated diabetes. [Conclusion] The investigation shows that the high risk factors for the nail-tract infection in the patients treated with external fixation apparatus are the nail placing time, severity of trauma, hormone applying and associated diabetes. The patients with risk factors may be monitored carefully in order to prevent the nail-related infection.
