经腹全胃切除术在晚期胃癌治疗中的应用体会

目的 探讨晚期胃癌经腹全胃切除术手术治疗的效果。方法 26例胃癌病人接受了全胃切除的手术治疗。其中胃小弯癌18例,胃体癌2例,胃底 胃体癌6例。结果 病理检查提示管状腺癌9例,低分化腺癌12例,中分化腺癌5例。所有病例有不同程度的胃周围淋巴结转移,无腹腔内重要脏器转移。结论 根据肿瘤部位选择适当的手术步骤,手术成功的要点之一是熟悉肠系膜动脉血管弓的解剖和变异。

Experience of abdominal total gastrectomy in curing the late gastric cancer

Objective To study operation technique and effect of abdominal total gastrectomy. Methods 26 cases of the late gastric cancer are presented. All patients were accepted the abdominal total gastrectomy. Results No operative death. Pathological examination revealed tubular adenocarcinoma in 9,poor differentiation in 12 and middifferentiation in 5. Conclusion Choice of proper operative procedure depend on the extent and location of the tumor. The key to operation should grasp the dissecting factor of mesenteric artery and prevent the anastomotic leakage effectively.
