某医院I类切口手术预防性使用抗菌药物情况分析

目的了解某医院I类切口手术预防使用抗菌药物情况。方法调查2005年1-12月出院的903例I类切口手术(乳腺、甲状腺手术)患者病历,对数据进行统计分析。结果I类切口手术预防用药率达100%,使用频度由高至低依次为青霉素类(455例次),第三、四代头孢菌素(232例次),喹诺酮类(132例次),第一、二代头孢菌素(77例次),克林霉素类(19例次);预防用药开始时间多在术前2~24h或术后;术后平均用药5.8d。结论该院I类切口手术预防用药存在指征把握不严、种类选择欠妥、用药时机不当以及过度用药等问题,亟需加强规范化用药管理。

Analysis on the usage of the prophylactic antimicrobial agents in patients with type I incision operation

Objective To survey the usage of prophylactic antimicrobial agents in patients with type I incision operation in a hospital.Methods Case history of 903 patients with type I incision operation(operation of mammary gland and thyroid gland) who discharged from hospital from January to December 2005 were investigated and analysed.Results The prophylactic antimicrobial usage rate was 100% in 903 patients.The most frequently used antimicrobial agents was penicillins(455 cases),followed by the third and fourth generation cephalosporins(232 cases),fluoroquinolone(132 cases),the first and second generation cephalosporins(77 cases) and clindamycin(19 cases).The major prophylactic antimicrobial agents were used 2~24 hours before operation and an average of 5.8 days after operation.Conclusion There are problems in prophylactic antimicrobial usage in patients with type I incision operation in this hospital,the indications are not stringent,category of the antimicrobial agents is not chosen properly;the using occasion is not appropriate,antimicrobial agents is excessively used.It is urgent to strengthen the management of standard antimicrobial usage.
