经皮椎体成形术和经皮椎体后凸成形术治疗骨质疏松性椎体压缩骨折

摘    要:目的评估经皮椎体成形术(PVP)和经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折的疗效。方法30例老年骨质疏松性胸腰椎压缩骨折患者随机分成PVP组和PKP组,PVP组7例单椎体,5例2个椎体、2例3个椎体、1例4个椎体,共27个椎体接受治疗;PKP组9例单椎体、5例2个椎体、1例3个椎体,共22个椎体接受治疗。结果30例患者手术操作全部成功。PVP组8例完全缓解,7例部分缓解;PKP组10例完全缓解,5例部分缓解。术后PKP组椎体压缩率和Cobb角与术前比较有改善(P〈0.05)。结论PVP和PKP两种手术方法均具有显著止痛效果,PKP较PVP在椎体高度恢复及脊柱后凸畸形矫正效果更好。

Abstract:Objective To evaluate the clinical effects of the pereutaneous vertebrophasty(PVP) and percutaneous kyphoplasty (PKP) in the treatment of the osteoporotic vertebral compression fracture. Methods Thirty patients with elderly osteoporotic compression fracture of thoracolumbar underwent PVP or PKP group (each n: 15). Results The operations were successfully performed. Complete remission and partial remission was obtained in 8 and 7 cases of PVP,while in PKP group in 10 and 5 cases, respectively. There was a significant difference in the vertebral compression rate and Cobb angle in PKP group before surgery and after surgery (P〈0.05). Conclusion PVP and PKP is of significant value on pain relief with minimally invasive feature. PKP does better than PVP on the recovering of vertebral body and correction of kyphosis.
