规范化辨证治疗寻常型银屑病200例临床疗效观察

目的观察采用规范化辨治方案治疗寻常型银屑病的临床疗效,探讨从"血"论治银屑病的辨治规律,为建立银屑病中医辨证规范提供依据。方法采用前瞻性、多中心、大样本、随机、单盲、对照试验研究方法 ,治疗组内服根据中医名家经验制定的标准方药,对照组内服接诊医师自行处方,疗程8周。治疗前后进行PASI评分,评价疗效,同时注意观察不良反应。结果治疗组200例,对照组213例。两组PASI评分、各单项症状评分在治疗后均较治疗前有显著性降低(P0.01或P0.05),但治疗后两组间比较,差异无统计学意义(P0.05);两组总体疗效、各证型疗效差异亦无统计学意义(P0.05)。两组均未见明显不良反应。结论从血论治银屑病,分为血热、血燥、血瘀三型,采用规范化辨治方案可行有效。

Clinical Therapeutic Observation of 200 Cases of Psoriasis Vulgaris Treated with Standardized Syndrome Differentiation

Objective To observe the clinical efficacy on psoriasis vulgaris treated with the program based on the standardized syndrome differentiation and probe into the rule of syndrome differentiation in treatment of the disease from ＂blood＂ so as to provide the evidences for the establishment of the standard syndrome differentiation of psoriasis vulgaris in traditional Chinese medicine（TCM）.Methods A prospective,multi-central,large sample,randomized,single blind and controlled trial was adopted in the research.In treatment group,the standard formula was used for oral administration,which was prescribed according to the experiences of eminent physicians in TCM.In control group,the formula was prescribed by the clinical physician him/herself for oral administration.The course of treatment was 8 weeks.PASI score and the efficacy were evaluated before and after treatment.At the same time,adverse reactions were observed.Results There were 200 cases in treatment group and 213 cases in control group.After treatment,PASI scores and each symptom score in two groups all decreased significantly as compared with those before treatment（P〈0.05）.There was no statistical significance of the differences in comparison between two groups（P〉0.05）.The differences in overall efficacy and the efficacy on each syndrome were not significant statistically between two groups（P〉0.05）.No obverse adverse reaction happened in two groups.Conclusion Based on blood therapy in treatment,psoriasis vulgaris is differentiated as blood heat,blood dryness and blood stagnation syndromes.The standardized syndrome differentiation is feasible and effective in the treatment.
