改良PPH术治疗重度痔病的临床疗效分析

目的探讨采用改良PPH术治疗重度痔病的临床效果。方法将2012年5月至2013年12月来本院就诊的112例重度痔病,按治疗方法分为对照组和观察组两组,每组56例;其中对照组采用传统痔上粘膜环切术,观察组行改良的痔上粘膜环切术;对比两组的手术情况、临床疗效、术后肛门疼痛及1个月内的并发症。结果观察组患者的手术时间、手术出血量及平均住院时间均显著性小于对照组（P〈0.05）;且术后30天对两组患者的术后并发症比较,结果显示,观察组肛门疼痛、肛缘水肿、肛门狭窄、肛门溢液的发生率显著性低于对照组（P〈0.05）,但在切口出血、尿潴留、痔核回缩率方面,两组比较无显著性差异（P〉0.05）;对所有患者随访6-12个月,对比两组患者术后满意度及临床疗效,结果显示,观察组的临床满意度显著性高于对照组（P〈0.05）,但临床疗效两组无差异。结论采用改良的PPH治疗重度痔病能够明显减少手术时间、减轻术后肛门疼痛、降低术后并发症的发病率,可作为治疗重度痔病的理想治疗方法。

Clinical analysis of using mPPH for the treatment of severe hemorrhoids

Objective To observe the clinical curative effect of using m PPH in the treatment of severe hemorrhoids.Methods From May2012 to December 2013 in our hospital 112 cases with severe hemorrhoids were collected and divided into two groups（56 cases in each group）according to the treatment method： the control group given traditional PPH and the observation group given modified PPH.Clinical curative effects,postoperative anal pain and complications within one month were recorded and analyzed.Results The operation time,surgical blood loss and the average hospital stay in the observation group were significantly less than those of the control group（P〈0.05）.And Results of complications 30 days after surgery showed that anal pain,anal edge edema,anal stricture,and anal discharge incidences in the observation group were significantly lower than those in the control group（P〈0.05）,but there were no differences in terms of incision bleeding,urinary retention and haemorrhoids retraction rate（P〉0.05）.All patients were followed up for 6-12 months,and the clinical satisfaction Results of the observation group were significantly higher than that of control group（P〈0.05）,and no difference was found in terms of clinical curative effect between the two groups.Conclusion Modified PPH to treat severe hemorrhoids sickness could significantly reduce the operation time,the postoperative anal pain and the postoperative complications incidence,which could be used as ideal therapy to treat severe hemorrhoids.
