血清vWF、IL-6、CRP与糖尿病患者血管病变的相关性分析

摘    要:目的探讨von Willebrand因子(vWF)、白细胞介素-6(IL-6)、C反应蛋白(CRP)与糖尿病患者血管损伤的关联性。方法检测各期糖尿病患者的空腹血糖(Glu)、糖化血红蛋白(HbA1c)、vWF、IL-6、CRP、尿白蛋白和血压,研究各期患者检查指标之间的关联及差异。结果随着糖尿病的发展,所有检测指标的均值表现出明显的增高趋势。糖尿病患者Glu的升高和CRP之间相关系数(r)为0.491,尿白蛋白异常组血vWF、IL-5也增高;肾功能不全组vWF升高与其他组有显著差异;虽然HbA1c与尿白蛋白的r值为0.413,具有相关性,但HbA1c×血压(收缩)与尿白蛋白的r值为0.469,其关联性更强。结论随着糖尿病病程的进展,多种炎症标志物也随之上升,支持了糖尿病被认为是一种炎症性疾病的观点。临床在对糖尿病患者进行血糖控制时,应同时加强对患者炎症反应的控制,以减缓血管的病理变化。IL-6、CRP、特别是vWF可作为糖尿病血管病变控制是否有效的监测手段。

Abstract:Objective To investigate the association of yon Willebrand factor (vWF) , C-reactive protein (CRP) and interlukin-6 (IL-6) with the vascular damage in diabetes patients. Methods Fasting glucose (Glu), glucosylated hemoglobin (HbA1 c), vWF, IL-6, CRP, urine albumin and blood pressure of various diabetic groups were measured. The relativity and differences between these data were analyzed. Results The mean values of all indexes showed obvious incremental tendency with the development of diabetes. The correlation coefficient of Glu and CRP was 0.491. The blood levels of vWF and IL-6 in the pationts with abnormal urine albumin increased. In the patients with kidney functional deficiency, vWF was significanly higher than in other patients. The correlation coefficient of HbA1c and urine albumin was 0.413 ,but the correlation coefficient of HbA1c×blood pressure and urine albumin was 0.469, which showed a stronger relationship. Conclusions The fact that many kinds of inflammatory factor could increase with diabetes progressing supported the viewpoint of considering diabetes as a kind of inflammatory disease. While a treatment to the diabetics is being carried out, the inflammatory reaction should be controlled strenuously so that the pathological change of blood vessel could be delayed. IL-6, CRP and specially vWF may be effective indexes to monitor the pathological vascular change of diabetics.
