初产妇头位难产诊断中新旧产程图的对比研究

目的探讨初产妇头位难产诊断中新旧产程图应用的对比研究。方法选取2013年9月至2015年8月在清华大学玉泉医院产科病房的212例头位难产初产妇,根据产程图分为新产程图组和旧产程图组,比较两组产程时间、产科干预措施、临床表现及妊娠结局。结果新产程图组第一产程和第二产程均较旧产程图组有显著变化,产科干预措施明显减少、剖宫产率有所降低,差异有统计学意义(P0.05);产妇临床表现两组差异无统计学意义(P0.05)。结论采用新产程图处理头位难产初产妇能降低剖宫产率、减少产科干预措施,有助于对初产妇头位难产做出及早的诊断。

Comparsion of new labor curve with Friedman curve in the diagnosis of cephalic presentation dystocia in primipara

Objective To assess the comparsion of new labor curve with Friedman curve in the diagnosis of cephalic presentation dystocia in primipara.Methods 212 primipara with cephalic presentation dystocia in Yuquan Hospital of Tsinghua University from September 2013 to August 2015 were selected and divided into two groups according to different labor curves they used.One group was evaluated with new labor curve, while the other group was evaluated with Friedman labor curve.The time of labor stage, intervening measure, clinical feature and pregnancy outcome of the two groups were compared.Results The first and second stages of the group evaluated with new labor curve were significantly different from the group evaluated with Friedman labor curve.The intervening measures were less used when in the group evaluated with new labor curve.Cesarean delivery rate also decreased in this group.Conclusion Using the new labor curve may decrease the cesarean delivery rate and other intervening measures in delivery.And the new labor curve is much more useful in the early diagnosis of cephalic presentation dystocia in primipara.
