新生儿缺氧缺血性脑病35例CT征象分析

摘    要:目的:探讨新生儿缺氧缺血性脑病在CT检查中的征象分析。方法收集我院近4年来35例新生儿缺氧缺血性脑病的CT资料,结合文献,分析新生儿缺氧缺血性脑病的CT征象。15例1叶脑组织小片状低密度影;13例2叶脑组织散在片状低密度影;7例灰白质界限不清,脑室受压变小,并伴有颅内出血(包括脑室内及室管膜下出血灶,蛛网膜下腔出血,硬膜下出血及脑内血肿)。16例为出生后1d,14例为出生后3d,5例为出生后4d。结果CT平扫加薄扫即可发现新生儿缺氧缺血性脑病;CT分度为:15例为轻度,13例为中度,7例为重度新生儿缺氧缺血性脑病。结论CT平扫加薄扫对新生儿缺氧缺血性脑病具有很高的敏感性,结合临床分期,明确其与CT分度之间的关系,预后判断,为临床治疗提供依据。

Abstract:Objective To investigate the CT signs of neonatal hypoxic ischemic encephalopathy. Methods The CT data of 35 cases with neonatal hypoxic ischemic encephalopathy were collected in our hospital within recent 4 years,and the CT signs were analyzed. There were 15 cases of small schistose low-density image in brain tissues of one lobe, 13 cases of scattered schistose low-density image in brain tissues of two lobes, 7 cases of unclear grey matter margin,smaller compressed ventricles and combined with intracranial hemorrhage(including intraventricular or subependymal hemorrhage, subarachnoid hemorrhage, subdural hemorrhage and encephalic haematoma).There were 16 cases who lived 1 day after birth,14 cases who lived 3 days after birth,and 5 cases who lived 4 days after birth. Results The neonatal hypoxic ischemic encephalopathy was found by CT scan combined with tlc scanning;The CT grading showed that 15 cases were mild,13 cases were moderate and 7 cases were severe neonatal hypoxic ischemic encephalopathy. Conclusion CT scan combined with tlc scanning has a high sensitivity in neonatal hypoxic ischemic encephalopathy, and combining with clinical stage can identify the relationship between it and CT grading, and judge the prognosis, and provide the basis for the clinical therapy.
