内生湿热证人群的肠道菌群特点研究

摘    要:目的探讨内生湿热证人群与健康人群的肠道菌群差异性。方法选取2019年4—9月于江苏省第二中医院门诊就诊及住院的24例内生湿热证患者作为湿热组,选取同期8~14岁健康男童20例作为对照组,取2组粪便样本,通过Illumina MiSeq平台对肠道菌群进行16S rDNA基因V3~V4可变区检测,运用QIIME、STAMP及LEfSe进行Alpha、Beta多样性分析,鉴定差异性菌属。结果湿热组的Shannon指数、Evenness指数均明显低于对照组(P均<0.05)。主坐标分析显示2组样本菌群结构分布差异有统计学意义(P<0.05)。门分类水平上,湿热组拟杆菌门与厚壁菌门的相对丰度比值呈下降趋势,疣微菌门的相对丰度比值明显低于对照组(P均<0.05);属分类水平上,湿热组布劳特氏菌属、迟缓埃格特菌的相对丰度均明显高于对照组(P均<0.05);瘤胃球菌属、双歧杆菌属、Alistipes、颤螺菌属、Odoribacter、Anaerostipes、无色杆菌属的相对丰度均明显低于对照组(P均<0.05)。结论内生湿热证患者存在肠道菌群紊乱,其发病主要与拟杆菌门与厚壁菌门的相对丰度比值下调,迟缓埃格特菌属丰度上升,瘤胃球菌属、双歧杆菌属、颤螺菌属丰度下降相关。

Abstract:Objective It is to investigate the differences of intestinal flora between patients with endogenous damp-heat syndrome and healthy people.Methods Twenty-four patients with endogenous damp-heat syndrome who were treated and hospitalized in the outpatient department of Jiangsu Second Hospital of Traditional Chinese Medicine from April to September 2019 were selected as the damp-heat group,and 20 healthy boys aged 8-14 were selected as the control group,and the stool samples were taken from the two groups,the variable region of 16S rDNA gene V3~V4 of the intestinal flora were detected through the Illumina MiSeq platform,QIIME,STAMP and LEfSe were used for Alpha and Beta diversity analysis to identify different bacterial genera.Results The Shannon index and Evenness index of the damp-heat group were significantly lower than those of the control group(all P<0.05).Principal coordinate analysis showed that the differences in the structure distribution of samples were statistically significant between the two groups(P<0.05).At the phylum classification level,the relative abundance ratio of Bacteroides and Firmicutes in the damp-heat group showed a decreased trend,the relative abundance of Verrucomicrobia in the damp-heat group was significantly lower than that of the control group(P<0.05);at the genus level,the relative abundances of Lauterella and Escherichia lentus of the damp-heat group were significantly higher than those of the control group(both P<0.05);the relative abundance of Rumencoccus,Bifidobacterium,Alistipes,Oscillatoria,Odoribacter,Anaerostipes,Achromobacter of the damp-heat group were significantly lower than those of the control group(all P<0.05).Conclusion There was gut microbiota imbalance in patients with endogenous damp-heat syndrome,which is mainly related to the decrease of the proportion of Bacteroidetes/Firmicutes,the increase of the abundance of Eggerthella,and the decrease of the abundance of Ruminococcus,Bifidobacterium and Oscillospira.
