进展期胃癌淋巴结转移影响因素的分析

目的探讨影响进展期胃癌淋巴结转移的因素，指导选择合适的治疗方案。方法回顾性分析96例进展期胃癌患者临床病理资料与淋巴结转移的关系。结果肿瘤直径〉5cm、3cm〈肿瘤直径≤5cm的淋巴结转移率分别为90．5％（38／42）、78．4％（29／37）明显高于肿瘤直径≤3cm的29．4％（5／17）（P〈0．01）；侵及邻近组织和器官、穿透浆膜层而未侵及邻近组织及器官、侵及浆膜层的淋巴结转移率分别为91．7％（11／12）、77．8％（21／27）、89．7％（26／29），高于侵及肌层的50．0％（14／28）（19〈0．01或P〈0．05）。性别、年龄、肿瘤部位以及组织学类型间的淋巴结转移率没有显著差异。结论肿瘤大小、浸润深度是影响进展期胃癌淋巴结转移的因素，应根据肿瘤大小和浸润深度选择合适的治疗方案。

Analysis of Risk Factors for Lymph Node Metastasis in Advanced Gastric Cancer

Objective To explore the risk factors for lymph node metastasis in advanced gastric cancer, and put forward suitable therapy for advanced gastric cancer. Methods The data of 96 patients with advanced gastric cancer were retrospectively analyzed with respect to the relationship between clinicopathological characteristics and lymph node metastasis. Results The lymph node metastasis rate of tumor diameter 〉 5cm （ 90.5% ）, and of 3cm 〈 tumor diameter ≤ 5cm （78.4%） were higher than that of tumor diameter ≤ 3cm （ 29.4% ） （ P 〈 0.01 ） ;The rate of lymph node metastasis of serosa invasion（ 89.7% ）,penetrate serosa invasion（77.8%） ,penetrate serosa and invade vicinity tissues（91.7% ） were significantly higher than that of lamina muscularis invasion（50.0% ） （P 〈 0.05 or P 〈 0.01 ）. The gender, age, tumor site and histological type were not correlative factor of lymphatic metastasis. Conclusion The tumor size,depth of invasion were risk factors for lymph node metastasis in advanced gastric cancer. According to tumor size, depth of invasion is essential to the appropriate therapeutic options for advanced gastric cancer.
