制动时间对兔膝关节软骨损伤及修复的影响

摘    要:目的探讨不同的制动时间对兔膝关节软骨损伤以及损伤后修复的影响。方法 49只雄性新西兰白兔随机分为7组(每组7只):A1组、A2组、A3组、B1组、B2组、B3组、C组。采用管型石膏将兔左后肢膝关节伸直位固定。A1组、A2组和A3组制动4周,B1组、B2组和B3组制动6周,A2组和B2组解除石膏后自主活动4周,A3组和B3组解除石膏后自主活动8周,C组不做任何处理。每组随机选取1只,进行HE和番红"O"染色观察,另外6只测量膝关节活动范围、观察软骨大体形态,采用Real-time PCR法测定软骨细胞Ⅱ型胶原以及蛋白聚糖mRNA基因表达。结果制动时间越长,膝关节活动度越小(P〈0.01),软骨退行性改变越严重,Ⅱ型胶原和蛋白聚糖mRNA基因表达越少(P〈0.01),解除制动后上述指标恢复速度越慢(P〈0.01)。结论软骨损伤随膝关节制动时间延长而加重;短期制动所造成的软骨损伤在解除制动并活动一定时期后可自行修复,修复速度随制动时间延长而减慢。
