幽门螺杆菌阳性十二指肠球部溃疡采用四联疗法治疗效果分析

目的采用四联疗法对于幽门螺杆菌阳性十二指畅球部溃疡进行治疗，对治疗效果进行分析。方法选择我院从2010年1月至2011年1月收治的幽门螺杆菌阳性十二指肠球部溃疡患者60例，随机分成两组，各30例，治疗组口服雷贝拉唑10mg，bid，雷尼替丁枸橼酸铋350mg，bid，阿莫西林1000mg，bid，呋喃唑酮100mg，hid，连续服药4d.对照组口服奥美拉唑20mg，bid．阿莫西林1000mg，bid，克拉霉素500mg，bid，连续服药7d．采取以上治疗后，两组患者均改用雷贝拉唑10mg，qd，连续服药14d。经过治疗后5周左有观察治疗效果，然后在1年以后进行随访。结果治疗组和对照组溃疡愈合率分别为96．67％，76．67％，Hp根除率分别为96．67％，86．67％，治疗组溃疡愈合率、Hp根除率明显高于对照组。随访发现治疗组溃疡复发率、Hp再感染率明显低于对照组，差异均有统计学意义（P〈0．05）。结论 采用四联疗法加上雷贝拉唑对于幽门螺杆菌阳性十二指肠球部溃疡进行治疗，治疗效果显著，值得临床推广。

Curative effect analysis on Helicobacter pylori positive duodenal ulcer by quadruple therapy

Objective To shuly the quadruple therapy for Helicohacter pylori positive duodenal ulcer treatment. the therapeutic effects were analyzed. Methods In our hospital from 2010 January to 2011 January 60 cases, were, treated for Helieobacter pylori positive duodenal ulcer, they were randomly diyvided into two groups. 30 cases in each group, the treatment group was treated with oral ray Bella of 10 rag, timeranitidine bismuth citrate 350 mg 1 time 1000 mgl times, amoxicillin, furazolidone 100 mg 1 time, taking 4 days. The control group treated by omeprazole 20 mgl, 1000 mgl 500 mgl amoxieillin, clarithromycin, medicationtime 1 day 2 times, a total of medication for 7 days. After treatment, patients of two groups also needed to take the ray Bella of 10mg 1 time, 1 time 1 day, a total of 2 weeks after taking. After treatment the curative effect was observed after 5 weeks, and after l year of follow-up. Results The rate of ulcer healing in the treatment group and the control group well 96.67% and 76.67% respectively, the Hp removing rate was 96.67% comparing with 86.67% of the control group. In the treatmenl group, the ulcer healing rate and the eradication rate of Hp were higher than those of control group. The treatment group was found to ulcer recurrence rate and Hp reinfection rate were lower than those of control group. There were both significant differences between the treatment group and eontrol group （P 〈 0.05）. Conclusion The quadruple therapy plus Reba La for Helieobacter pylori positive duodenal ulcer treatment, has remarkable treatment effect, worthy of clinical application.
