鼻咽癌放射治疗后诱发的第二恶性肿瘤的临床特点分析

目的 探讨鼻咽癌放射治疗后诱发的第二恶性肿瘤的临床特点.方法 回顾分析28例鼻咽痛患者因放射诱发的第二原发性恶性肿瘤的临床资料.结果 本组28例患者中,鳞状上皮癌16例,占57.1%;肉瘤类5例,占17.9%;腺样囊性癌3例,占10.7%;基底细胞癌3例,占10.7%;恶性黑色素瘤1例,占3.6%.经手术治疗20例,3年生存率75%,5年生存率35%;放疗结合化疗的3年生存率25%;单纯放疔或化疗的,均于2年内死亡.结论 放射诱发的第二原发性恶性肿瘤如具备手术适应证,则应首选手术或包含手术的综合治疗.积极治疗第二原发性恶性肿瘤同样可以获得较好的治疗效果.

Clinical characteristics of second primary carcinoma induced by radiation therapy for nasopharyngeal carcinoma

Objective To investigate the clinical characteristics of second primary carcinoma induced by radiation therapy in patients with nasopharyngeal carcinoma. Methods A retrospective analysis was carried out to investigate the clinical data of radiation-induced second primary carcinoma in 28 cases with nasopharyngeal carcinoma. Results Among 28 cases, 16 (57.1%) cases presented squamous cell carcinoma; 5 (17.9%) cases presented sarcoma; 3 (10.7%) presented adenoid cystic carcinoma; another 3 (10.7%) presented basal cell carcinoma; and 1( 3.6%)presented malignant melanoma. In 20 cases that accepted surgical treatment, the 3-year, and 5-year survival rate were 75% and 35%; the 3-year survival rate after radiation therapy combined with chemotherapy was 25%; those with only radiotherapy or chemotherapy were all dead in 2 years. Conclusions If a patient with radiation-induced second primary carcinoma has an indication for surgery, it' s better to use treatment such as surgery or comprehensive treatment. The positive treatment to second primary carcinoma can also get good curative effect.
