重症急性胰腺炎并发多脏器功能失常综合征时内皮素和一氧化氮的变化及药物的治疗作用

目的：探讨重症急性胰腺炎（ＳＡＰ）并发多脏器功能失常综合征（ＭＯＤＳ）时内皮素（ＥＴ）和一氧化氮（ＮＯ）的变化及头孢噻甲羧肟（复达欣）与复方丹参滴丸的治疗作用。方法：采用５％牛磺胆酸钠逆行胆胰管注射建立大鼠ＳＡＰ并发ＭＯＤＳ模型；观察血浆ＥＴ和血清ＮＯ含量变化，肝、肾、胰腺功能改变以及内脏组织的病理变化；同时观察复达欣与复方丹参滴丸的治疗作用。结果：ＳＡＰ并发ＭＯＤＳ时，血浆ＥＴ和血清ＮＯ含量升高，肝、肾、胰腺功能衰竭，内脏组织病理损害严重；经药物治疗后，血浆ＥＴ和血清ＮＯ含量有所减少，肝、肾、胰腺功能有改善，内脏组织病理损害减轻。结论：ＳＡＰ并发ＭＯＤＳ时内皮细胞功能受损，表现为血浆ＥＴ和血清ＮＯ产生增多；复达欣和复方丹参滴丸对ＳＡＰ并发ＭＯＤＳ有治疗作用

Changes in endothelin and nitric oxide in severe acute pancreatitis complicated by multiple organ dysfunction syndrome

Objective:To study the role of endothelin (ET) and nitric oxide (NO) in severe acute pancreatitis(SAP) complicated by multiple organ dysfunction syndrome (MODS),and to observe the effects of ceftazidime (Fortum) or compound Danshen dropping pills (CDSDP) on SAP.Methods:The SAP model was induced by retrograde injection of 5% sodium taurocholate solution into biliopancreatic duct,the contents of ET,NO,glutamic pyruvic transaminase (GPT),blood urea nitrogen (BUN) and amylase (Amy) were measured,histological changes in liver,lung and pancreas were also examined at 12 hours after induction of SAP.Results:ET and NO levels markedly increased,and the contents of GPT,BUN and Amy were higher in salinetreated group than those in other groups.The pathological changes of visceral tissues were severe in salinetreated group,which were ameliorated by treatment with Fortum or CDSDP.Conclusions:Endothelial cell dysfunction,leading to overproduction of ET and NO,may have an important role in the development of MODS associated with SAP.Fortum and CDSDP can be useful in the treatment of SAP complicated by MODS.
