铜绿假单胞菌氨基糖苷类药物产酶耐药机制研究

摘    要:目的了解我院临床分离的30株多重耐药铜绿假单胞菌中16SrRNA甲基化酶基因及氨基糖苷类修饰酶(AMEs)基因存在状况。方法收集山东和江苏两地医院临床分离的铜绿假单胞菌共50株(山东烟台多重耐药30株,江苏泛耐株20株),采用聚合酶链反应(PCR)及序列分析的方法分析6种16SrRNA甲基化酶(armA、rmtA、rmtB、rmtC、rmtD、npmA)和6种AMEs基因aac(3).Ⅰ、aac(3)-Ⅱ、aac(6’)-Ⅰb、aac(6’)-Ⅱ、ant(3″)-Ⅰ和ant(2″)-Ⅰ。结果山东烟台30株多重耐药铜绿假单胞菌中4种基因aac(3)-Ⅱ、aac(6')-Ⅱ、ant(3″)-Ⅰ和ant(2″)-Ⅰ的阳性株数分别为6株(20%)、14株(46.7%)、11株(36.7%)和1株(3.3%),其余8株基因均为阴性;AMEs基因总阳性率为50%(15/30)。江苏20株泛耐株中6种基因rmtB、aac(3)-Ⅱ、aac(6')-Ⅰb、aac(6’)-Ⅱ、ant(3’r)-Ⅰ和ant(2″)-Ⅰ的阳性株数分别为11株(55%)、12株(60%)、7株(35%)、12株(60%)、10株(50%)和9株(45%),其余6株基因均为阴性;AMEs基因总阳性率为95%(19/20)。结论铜绿假单胞菌存在16SrRNA甲基化酶基因为国内首次报道;铜绿假单胞菌AMEs基因携带率很高;二家医院检出酶基因种类有差别。

Research of mechanism of enzyme-producing and drug resistance of pseudamonas aeruginosa

Abstract:Objeetive To investigate the 16SrRNA methylases gene and AMES of 30 strains of multi-resistant pseudomonas aeruginosa isolatted from patients in our hospital. Methods Collecting 50 strains of pseudomonas aeruginosa from two hospitals(30 strains of multi-resistant pseudomonas aeruginosa from Yantai,Shandong,20 strains of pan-drugresistant from Jiangsu), and analysing 6 kinds of 16SrRNA methylases (armA, rmtA, rmtB, rmtC, rmtD, npmA) and 6 kinds of AMEs gene aac(3)-Ⅰ ,aac(3)-Ⅱ, aac(6')- Ⅰ b, aac(6')- Ⅱ, aac(3″)- Ⅰ ,and aac(2″)- Ⅰ by PCR and sequence analysis. Results In 30 strains of multi-resistant pseudomonas aeruginosa from Yantai, the positive rate of 4 kinds of genes aac(3)-Ⅱ, aac(6')-Ⅱ, aac(3″)-Ⅰ, and aac(2″)-Ⅰ were 20%(6/30), 46.7%(14/30), 36.7%(11/30) and 3.3%(1/30) separetely. The 8 kinds of the rest genes were all tested negative. The total positive rate of AMEs gene was 46.7% (14/30). In 20 strains of pandrugresistant from Jiangsu, the positive rate of 4 kinds of genes aac(3)- Ⅱ, aac(6')-Ⅱ, aac(3″)-Ⅰ, and aac(2″)-Ⅰ were 55%(11/20), 60%(12/20), 35%(7/20) and 60% (12/20), 50%(10/20), 45% (9/20) separetely. The 6 kinds of the rest genes were all tested negative. The total positive rate of AMEs gene was 95% (19/20). Conclusions It is the first report that 16SrRNA methylases gene existed in pseudamonas aeruginosa; There was very high positive rate of AMEs genotypes in pseudamonas aeruginosa; There was different gene from two hospitals.
