ICU患者早期肠内营养临床效果观察

目的 观察早期肠内营养（EN）用于ICU危重症患者的临床效果.方法 将106例ICU危重症患者随机分为治疗组和对照组各53例.治疗组采用早期EN,对照组采用传统胃肠营养.治疗2周后比较2组血清白蛋白（ALB）、血红蛋白（Hb）、体质量指数和胃肠道并发症发生情况.结果 营养支持前2组ALB、Hb及体质量指数差异无统计学意义（P＞0.05）;营养支持2周后,2组ALB、Hb及体质量指数均高于营养支持前,且治疗组优于对照组,差异均有统计学意义（P＜0.05）.治疗组胃肠道呕吐、腹胀、腹泻、便秘和上消化道出血发生率均低于对照组,差异均有统计学意义（P＜0.05）.结论 危重症患者早期给予EN可更好地改善全身营养状况,降低胃肠道并发症及出血等发生率,从而明显改善预后,且经济安全,值得临床推广应用.

The clinical efect observation of early enteral nutrition in ICU

Objective To analysis the clinical effect of EN in critically ill patients of ICU. Methods 106 cases of critically ill patients were randomly divided into treatment group and control group, each of 53 cases. Treatment group was trea- ted by early enteral nutrition, control group was treated by traditional enteral nutrition. After two weeks of nutritional support, compared the ALB,Hb,body mass index and the gastrointestinal complications. Results After two weeks of nutritional sup- port, the level of ALB,Hb and body mass index of 2 groups were higher than that before nutritional support, and the level of ALB ,Hb and body mass index of treatment group was higher than that of control group,the difference was statistically signifi- cant （ P 〈 0.05 ）. the incidence of vomiting, bloating, diarrhea, constipation, upper gastrointestitutl bleeding of treatment group was lower than that of control group, the difference was statistically significant （ P 〈 0.05 ）. Conclusion EN in critically ill pa- tients can improve the nutritional status, reduce the incidence of gastrointestinal complications and bleeding. Worthy of clinical application.
