急性有机磷中毒的血液灌流治疗

目的:评价血液灌流(HP)治疗急性有机磷中毒(AOPP)的价值。方法:67例AOPP患者分为两组,灌流组39例在传统治疗基础上行血液灌流治疗,对照组28例行传统治疗。比较两组间的治疗差异,比较灌流组患者灌流前后胆碱酯酶活力、格拉斯哥昏迷评分及血常规的变化。结果:灌流组的ICU停留时间、昏迷持续时间、阿托品累积用量、机械通气发生率及死亡率均小于对照组,有显著差异(P<0.01)。灌流组患者灌流前后胆碱酯酶活力、血红蛋白、白细胞无显著差异;格拉斯哥昏迷评分比灌流前显著升高(P<0.05);治疗后5d,灌流组胆碱酯酶活力明显高于对照组(P<0.05)。结论:HP治疗AOPP安全而有效。

Hemoperfusion in treating acute organophosphate poisoning

Objective: To evaluate the efficacy of hemoperfusion(HP) in treating the patients with acute organophosphate poisoning.Methods: Sixty-seven patients with acute organophosphate poisoning were divided into two groups,39 patients were treated with HP and traditional treatment(HP group),28 patients accepted traditional treatment(control group).The treatment effects were compared between the two groups and cholinesterase activity,Glasgow coma score and blood routine were determined before and after HP.Results: In the HP group,the ICU stay,duration of coma,cumulative dosages of atropine required,incidence of mechanical ventilation and mortality rate were significantly lower than that in the control(P<0.01).The cholinesterase activity,HGB and WBC were no significant difference before and after HP.The Glasgow coma score after HP was significantly higher than that before(P<0.05).In the HP group,the cholinesterase activity was significantly higher than that in the control after 5 days therapy(P<0.05).Conclusion: HP is effective and safety in treating acute organophosphate poisoning.
