闭合性胰腺外伤18例诊治分析

目的探讨闭合性胰腺外伤的诊断和治疗方法。方法对我院1991年6月至2006年6月收治的18例闭合性胰腺外伤的病例资料进行总结分析。结果18例中治愈17例,死亡1例。发生严重并发症6例(33.3%),其中胰瘘2例,腹腔脓肿1例,假性胰腺囊肿2例,假性胰腺囊肿伴大出血1例。结论早期腹部CT检查是诊断胰腺损伤最快捷、直观的方法,在剖腹探查术中常规探查胰腺是早期发现和避免漏诊胰腺外伤的关键。治疗时应综合考虑胰腺损伤的类型、合并伤和病人全身情况,选择合理的治疗方法。

Analysis of diagnosis and treatment of closed pancreatic trauma in 18 cases

Objective To study the diagnosis and treatment of closed pancreatic trauma.Methods Retrospectively analysis was carried in 18 closed pancreatic trauma cases from 1991 to 2006.Results In all cases,17 cases cured and 1 died,Severe complication were happened in 6 cases(33.3%),included 2 cases of pancreatic fistula,1 cases abdominal abscess,2 cases pancreatic pseudocyst,and 1 case pancreatic haemorrhage.Conclusion Early abdominal CT examine is the direct and simple way to diagnose pancreatic trauma,check-up pancreas as a routine in exploratory laparotomy can be key to early diagnosis and avoid misdiagnosis.Treatment methods should be made with the severity of pancreatic trauma,associated injury and general situation.
