高频超声与MRI对类风湿性关节炎腕关节骨质侵蚀的比较研究

目的 探讨高频超声对类风湿关节炎（Rheumatoid Arthritis,RA）腕关节骨质侵蚀的诊断价值.方法 30例RA患者纳入研究组,对其60个腕关节分别做高频彩色多普勒超声与MRI检查,并与健康组对比分析.结果 高频彩色多普勒超声检出研究组腕关节中480块腕骨中81块出现骨侵蚀,占16.9％,300个掌腕关节面中69个掌骨底发生骨质侵蚀,占23.0％,其中第二掌骨底32个,占10.7％;第三掌骨底24个,占8.0％;第四掌骨底13个,占4.3％.MRI检出100块腕骨骨侵蚀,占20.8％,掌腕关节处71块掌骨底出现骨质侵蚀,占23.3％.其中第二掌骨底33个,占11.0％;第三掌骨底25个,占8.3％;第四掌骨底11个,占3.7％;第5掌骨底1个,占0.01％.尺骨远端骨质侵蚀1个,占1.7％.结论 高频彩色多普勒超声能准确显示RA腕关节骨质侵蚀.

Diagnostic Value Comparative Study of High-frequency Ultrasound and MRI on Bone Erosion of Wrist in Rheumatoid Arthritis

Objective The study is to evaluate diagnostic value of high frequency ultrasound wrist bone erosion in Rheumatoid Arthritis （RA）. Methods 30 cases of RA patients were recruited into the study group, and the 60 wrist joints were examined respectively for high frequency color doppler ultrasound and MRI, analysis was carried out after comparing with healthy group. Results High frequency color doppler ultrasound detection in the group at the wrist, 300 pieces of 69 pieces in carpal bone erosion（23.0 % ）, MRI detected 100 pieces of carpal bone erosion, accounted for 20.8 %. Conclusion High frequency color doppler ultrasound can accurately display the RA wrist bone erosion.
