辛伐他汀对维持性血液透析患者微炎症状态的改善及其作用机制

目的探讨辛伐他汀在维持性血液透析患者微炎症状态中的改善作用及其机制。方法2009年1月至2011年6月，收集158例入住我院维持性透析患者，随机分为两组，一组在常规治疗基础上予辛伐他汀治疗作为观察组，一组仅予常规治疗作为对照组，检测两组患者应用前后血清hs—CRP（超敏C反应蛋白，high sensitive c—reactive protien）、IL-6（白介素-6，interleukin-6）、TNF-α（肿瘤坏死因子α，tumor necrosis factorct）水平的变化。结果MHD患者应用辛伐他汀治疗前CRP水平（11．23±6．95）mg／L，治疗3个月后为（3．41±2．24）mg／L，二者比较差异具有统计学意义（P〈0．05）。同时MHD患者应用辛伐他汀治疗3个月前后血清IL-6水平、TNF-α水平分别比较，差异具有统计学意义（P〈0．05）。治疗前观察组血清CRP、IL-6、TNF-α水平与对照组相比，差异无统计学意义（P〉0．05）。治疗3个月后，观察组血清CRP、IL-6、TNF-α水平与对照组相比较明显下降，差异具有统计学意义（P〈0．05）。结论维持性血液透析患者应用他汀类药物治疗可有效降低微炎症状态水平，可能机制在于其抑制了前炎症细胞因子的激活。

Effect of Simvastatin in improving the micro-inflammatory state of patients treated with maintenance hemodialysis and its action mechanism

Objective To investigate the effect of Simvastatin on microinflammatory state in maintenance hemodialysis patients.Methods From January 2009 to June 2011,158 cases maintenance hemodialysis were recruited and divided randomLy into two groups as observation group and control.The observation group accepted Simvastatin treatment beside regular therapy.And the control group accepted regular therapy only.The microinflammatory state indexes including serum CRP,IL-6 and TNF-α level before and after the treatment were detected and analyzed by t test in SPSS 17.0 for windows.Results After 3 months of Simvastatin medication,the observation group CRP level,IL-6 level and TNF-α level was obviously decreased(P <0.05) as compared with those before the treatment.Compared to the control,the microinflammatory state indexes including serum CRP,IL-6 and TNF-α level of observation group after the treatment had significant difference.Conclusion Simvastatin therapy in maintenance hemodialysis patients could decrease microinflammatory state indexes,which might prevent hemodialysis related complications from further developing.
