超脉冲CO2点阵激光联合重组牛碱性表皮生长因子凝胶治疗面部术后凹陷性瘢痕的效果及安全性

摘    要:目的探讨超脉冲CO2点阵激光联合重组牛碱性表皮生长因子凝胶治疗面部术后凹陷性瘢痕的效果及安全性。 方法选取面部术后凹陷性瘢痕患者100例,根据就诊病例号分为观察组和对照组。对照组给予超脉冲CO2点阵激光治疗,2个月治疗1次,5次为1个疗程;观察组在对照组基础上联合重组牛碱性表皮生长因子凝胶治疗,每次激光治疗后应用重组牛碱性成纤维细胞生长因子凝胶20 d,2次/d。1个疗程结束后比较2组疗效、创面结痂时间、红斑持续时间、创面疼痛时间、皮肤干燥紧绷感情况、患者满意度及治疗期间感染、水肿、新生瘢痕等不良反应发生情况。 结果2组治疗总有效率差异无统计学意义(P>0.05);观察组结痂时间、红斑持续时间及疼痛时间均短于对照组,皮肤干燥紧绷感发生率低于对照组,患者满意度高于对照组,治疗期间总不良反应发生率低于对照组,差异有统计学意义(P<0.05)。 结论超脉冲CO2点阵激光联合重组牛碱性表皮生长因子凝胶治疗面部术后凹陷性瘢痕,虽然未能提高疗效,但能加速创面修复,减轻创面疼痛、皮肤干燥紧绷等不适感,提高患者满意度,减少治疗期间不良反应发生率。

Abstract:ObjectiveTo investigate the efficacy and safety of ultra pulse CO2 fractional laser combined with recombinant bovine basic epidermal growth factor gel in the treatment of facial depressed scar after surgery.MethodsA total of 100 patients with depressed facial scar after surgery were selected and divided into observation group and control group according to patient number. The control group was treated with ultra pulse CO2 fractional laser once every 2 months, for a total of 5 times as 1 course of treatment, and the observation group was supplemented with recombinant bovine basic epidermal growth factor gel on the basis of the control group. After each laser treatment, recombinant bovine basic fibroblast growth factor gel was applied twice a day for 20 d. After one course of treatment, the curative effect, duration of wound scab formation, erythema duration, duration of wound pain, skin dryness and tension, patient satisfaction and adverse reactions such as infection, edema and new scar were compared between two groups.ResultsThere was no significant difference in the total effective rate betweentwo groups(P>0.05). Duration of wound scab formation, erythema and wound pain was shorter in the observation group than in the control group, and the incidence of skin dryness and tension was lower than that of the control group; the patient satisfaction was higher than that of the control group, and the total incidence of adverse reactions during treatment was lower than that of the control group, suggesting significant difference(P<0.05).ConclusionThe combination of ultra pulse CO2 fractional laser and recombinant bovine basic epidermal growth factor gel in the treatment of depressed facial scar after surgery failed to improve the curative effect, which, however, could accelerate wound repair, relieve wound pain and discomfort such as skin dryness and tension, improve patient satisfaction and reduce the incidence of adverse reactions during treatment.
