充气法MSCT动态增强对胃癌的诊断价值

目的探讨全胃充气法多层CT动态增强在胃癌诊断中的应用价值。方法上消化道钡餐及胃镜检查拟诊胃癌17例患者,以充气法行全胃16层CT增强扫描检查,并同手术病理对比。结果17例患者均行部分或全胃切除及区域淋巴结清扫术,病理证实胃腺癌15例,其术前全胃充气法多层CT增强扫描中局限隆起病灶11例,弥漫性增厚病变4例,病灶之CT呈现持续均匀强化,强化程度以门静脉期达最高;黏液腺癌2例,病变主要位于胃窦部,胃壁弥漫性增厚且密度不均匀,CT增强动脉期强化不明显,门静脉期呈“三明治”样强化表现。结论全胃充气法MSCT动态增强扫描对胃癌定位、定性诊断具有很高价值。

Diagnosis Value of Intubation Contrast-enhanced MSCT Examination in Gastric Carcinoma

Objective To investigate the method of dynamic contrast-enhanced MSCT examination in gastric cancer diagnosis.Methods 17 cases of gastric cancer patients will perform upper gastrointestinal barium meal and gastroscope is scanned by 16-slice enhancing CT by inflating method,and compared with their surgical pathological data.Results 17 cases of patients were partial or total gastrectomy and regional lymphadenectomy.Surgical pathology showed 15 cases of adenocarcinoma,among them,according to preoperative gastric inflatable multi-slice CT,11 cases showed elevated lesions,4 cases showed diffused thickening lesions,CT scan showed gastric adenocarcinoma uniformly continuous enhancement,with the highest appeared in portal vein maximum period;2 cases of mucinous adenocarcinoma,lesions are mainly located in the antrum,with diffuse gastric wall thickening and uneven density,CT arterial phase strengthening is not obvious,portal venous phase was a sandwich-like enhancement.Conclusion The methods of the stomach inflated dynamic contrast-enhanced multislice spiral CT scan have a significant values on the location of gastric cancer,the qualitative diagnosis.
