结节性甲状腺肿声像图特征与病理对照分析

目的：分析结节性甲状腺肿声像图特征，并进行超声分型，旨在提高对结节性甲状腺肿的诊断准确率尤其是甲状腺内单发结节的诊断。方法：对49例结节性甲状腺肿声像图特征与病理对照分析。结果：（1）孤立结节型12例，超声诊断正确率50%；（2）多发结节型11例，超声诊断正确率63.6%；（3）弥漫结节型26例，超声诊断正确率100%，结论：91）甲状腺内多个实性及囊实混合性结节病变，首先考虑为结节性甲状腺肿可能；（2）甲状腺内单个边界尚清的囊实混合性小结节，应考虑为结节性甲状腺肿可能；（3）甲状腺内较大的单个囊性，囊实混合性或实性结节病变，须根据结节内部回声，囊壁厚度，液性暗区内有无纤维光带间隔，包膜是否连续，边界清晰度，有无声晕，后方回声有无衰减等情况判断单个结节的甲状腺肿。

Analysis of the Contrast Between the Characteristics of Ultras ound image of Nodular Goiter and Pathology

Objective:In order to promote the diagnostic accordance rate of the nodular goiter,and the isolated node in thyroid especically,we analyzed the ultrasound images of nodular goiters and classified by ultrasound image features.Methods:To analyze the contrast between the characteristics of images and pathologic results of 49 cases with nodular goiters.Results:Among 12 cases of isolated node type,ultrasound diagnostic accordance rate is 50%.Among 11 cases of multiple node type,is 63 6%.Among 26 cases of diffuse node type,is 100%.Conclusions:Solid and mixed nodular lesions in thyroid should be considered as nodular goiter first.Small single nodules with mixed echoes and clear borders should be diasnosed as nodular goiters.In the situation of single cystic or mixed echo or solid nodules,the diagnosis of the goiter should be based on the concrete characteristics of the images such as internal echoes,thickness of the cystic wall,the dark liquid area with or without fibrosis,the continuity of capsules,the clearness of the boundary,the echo halo and rear attenuation.
