倍他乐克对慢性充血性心力衰竭患者心率变异性的影响

目的:探讨倍他乐克对慢性充血性心力衰竭(CHF)患者心率变异性(HRV)的影响。方法:70例CHF患者随机均分为2组,每组35例。2组患者性别、年龄及心功能差异无统计学意义。对照组给予常规抗心衰治疗,治疗组常规抗心衰治疗,在心衰初步控制后加服倍他乐克,从小剂量开始,缓慢增加剂量。治疗前和治疗3个月后分别测定2组患者的HRV;出院后随访半年,记录再住院率、病死率。结果:治疗3个月后,治疗组的HRV各指标SDNN、SDANN、rMSSD、PNN50均较治疗前及对照组明显提高(P<0.05),总有效率97.1%(34/35),高于对照组的71.4%(25/35)(P<0.05)。随访半年,治疗组再住院率17.1%(6/35),低于对照组42.9%(15/35)(P<0.05)。结论:在常规治疗的基础上,加用倍他乐克治疗慢性CHF,可提高患者HRV,增强疗效,改善预后。

Effect of betaloc on heart rate variability of patients with chronic congestive heart failure

Aim: To investigate the functional changes of cardiac autonomic nervous in patients with the treatment of betaloc. Methods: Patients in the control group got the traditional treatment against heart failure.Based on that,patients in the therapeutic group took betaloc additionally.Began with the minimum dosage,and then increased the dosage gradually.After 3 months' treatment, parameters of HRV were analyzed before and after treatment.Results: Compared with the control group,these HRV indexes of SDNN,SDANN,rMSSD,PNN50 of the therapeutic group all increased greatly(P<0.05).The cardiac function and the quality of life had improved.Furthermore the rates of mortality and re-hospitalization were less than those of the control group after 6 months' follow up.Conclusion: Based on the traditional therapy,taking betaloc additionally to treat chronic congestive heart failure can increase the patients' HRV,enforce the treatment effect and improve the prognosis.
