急性肾损伤相关危险因素及预后分析

目的:探讨住院患者急性肾损伤(AKI)的发病相关危险因素及其预后。方法:回顾性分析2008年12月～2011年12月期间我院肾内科住院416例AKI患者资料,将相关因素分别作关于病死率和肾功恢复的二分类非条件Logistic回归分析。结果:感染、血压控制不佳及肾毒性药物是AKI重要病因;呼衰、少尿、消化道出血、尿素氮是AKI病死率的危险因素,肾脏替代治疗可以降低AKI病死率;尿素氮、肾脏替代治疗、少尿及呼衰是影响肾功恢复的危险因素;肾功能恶化程度是肾功能是否完全恢复的影响因素之一。结论:重视AKI发生的病因、影响预后的危险因素,对于早期诊断及防治AKI极其重要。

Risk factors for acute kidney injury and its prognosis

Objective: To investigate the risk factors for acute kidney injury(AKI) and its prognosis.Methods:Data of 416 AKI patients that were treated in our hospital from Dec.2008 to Dec.2011 were analyzed.Unconditional binary logistic regression analysis was used for analyzing risks related to mortality and recovery of kidney function.Results:With multiple logistic analysis,we found that infection、uncontrolled blood pressure and the correlative medications harm to kidney was the important clinical causes for AKI;respiratory failure,urine shortage,limentary canal bleeding and bun were risk factors affecting the mortality of AKI patients,favorable factors which reduced the mortality of AKI was kidney substitution treatment;bun,kidney substitution treatment,urine shortage and respiratory failure were the risk factors affecting the recovery of kidney functions;severity of exasperate kidney function was one factors affecting complete recovery of kidney's function.Conclusions: Awareness of risk factors affecting the occurrence and prognosis of AKI is important for early diagnosis and treatment of the disease.
