小剂量低分子肝素治疗妊娠中晚期羊水过少的临床研究

目的:观察小剂量低分子肝素治疗妊娠中晚期羊水过少的临床疗效。方法;96例患者随机分为治疗组和对照组各48例,对照组采用静脉补液的传统治疗方法,治疗组则在补液的同时使用小剂量低分子肝素。治疗后复查羊水指数(AFI),脐动脉血流阻力(S/D比值),分娩时观测新生儿体重、新生儿Apgar评分、剖宫产率,测量两组患者产后出血量和产后出血的发生,观察治疗组凝血指标变化。结果:治疗后治疗组的AFI较对照组明显增多,S/D比值下降,差异有统计学意义(P<0.05);治疗组的剖宫产率较对照组低,治疗组新生儿体重较对照组重,新生儿Apgar评分好于对照组,差异有统计学意义(P<0.05);两组患者产后出血量和产后出血的发生率比较差异无统计学意义;治疗前后治疗组患者凝血相关指标比较差异无统计学意义,未发现治疗引起的凝血功能障碍。结论:小剂量低分子肝素治疗妊娠中晚期羊水过少疗效确切且安全,有利于改善母婴结局,具有临床推广使用的意义。

A clinical efficacy study on oligohydramnios in mid-or late pregnancy with low molecular weight heparin

Objective:To observe the clinical efficacy of low molecular weight heparin on oligohydramnios in mid-or late pregnancy. Methods:96 patients with oligohydramnios were randomly divided into treatment or control groups with 48 patients of each group.Patients in control group were given with traditional fluid infusion by vein while patients in treatment group were given with low molecular weight heparin in addition to traditional fluid infusion.Amniotic fluid index(AFI),fetal umbilical artery systolic pressure and diastolic pressure ratio(S/D),neonatal body weight,Apgar scores,percent of caesarean section of patients and postpartum hemorrhage between the two groups were compared.Blood coagulation indication of treatment group were observed. Results:Compared with control group,AFI,neonatal body weight and neonatal Apgar score were increased in treatment group(P<0.05),while S/D ratio and percent of caesarean section were decreased in treatment group(P<0.05).There was no significant difference in postpartum hemorrhage between treatment and control groups(P>0.05).No significant difference in blood coagulation indication was noticed in treatment group of patients before and after treatment. Conclusion:Low molecular weight heparin on oligohydramnios in mid-or late pregnancy seems effective and safe.It could improve pregnancy outcome.
