改良根治术与保乳手术治疗早期乳腺癌的近远期疗效比较

目的 探讨改良根治术与保乳手术治疗早期乳腺癌的近、远期疗效.方法 回顾性分析住院治疗的120例早期乳腺癌手术患者,其中改良根治术组52例,保乳手术组68例.比较两组患者手术效果、1年和5年生存率、局部复发率、远行转移率以及生活质量.结果 保乳手术组手术时间、术中出血量及住院时间显著少于改良手术组(P<0.05);两组患者1年及5年生存率、局部复发率、远行转移率均无显著差异(P>0.05);术后5年,保乳手术组SF-36各项评分显著高于改良手术组(P<0.05).结论 保乳手术与改良根治术治疗早期乳腺癌疗效确切,但保乳手术对患者损伤小、恢复快,术后患者生活质量高.

Short term and long term effect comparison of modified radical mastectomy and breast-conserving surgery in early breast cancer

Objective To explore the short term and long term effect comparison of breast-conserving surgery and modified radical mastectomy in early breast cancer.Methods The data of 120 cases of early breast cancer who underwent operation were retrospectively analyzed.The patients were divided into modified radical mastectomy group（n=52） and breast-conserving surgery group（n=68）.Operative effect,1 year and 5 year survival rate,local recurrence rate,metastasis rate and life quality were compared between two groups.Results Operation time,intraoperative blood loss and hospital stay in breast-conserving surgery were much less than those in modified radical mastectomy（P0.05）.There were no significant difference in 1 year and 5 year survival rate,local recurrence rate and metastasis rate between two groups（P0.05）;SF-36 scores of breast-conserving surgery 5 years after operation were much higher than modified radical mastectomy（P0.05）.Conclusion Modified radical mastectomy and breast-conserving surgery both have obvious curative effect in early breast cancer,but breast-conserving surgery has the advantages of less injury,faster recovery and better life quality.
