CT对亚急性甲状腺炎的诊断价值

目的探讨CT对亚急性甲状腺炎（SAT）的诊断价值，提高对该病的认识诊断能力。方法回顾性分析9例经手术、病理及实验室检查确诊的SAT螺旋CT平扫和增强扫描的cT表现。评估内容包括病灶部位、大小、形态、密度、边缘、增强程度、腺体边缘及颈部淋巴结情况。结果依病变形态分为弥漫型和局限型。弥漫型5例，分布于两侧甲状腺，同时累及峡部4例。呈弥漫不均匀低密度，增强扫描均呈明显不均匀强化。局限型3例，其中一侧单发结节1例，两侧多发结节2例，累及峡部1例。结节大小0.7-2.2cm，密度多低于骨骼肌，边缘模糊。增强扫描均呈明显或轻度不均匀强化，强化边缘不清。局限与弥漫型并存1例。所有甲状腺体积均增大，腺体边缘清晰，颈部未发现增大淋巴结。结论SAT的CT表现具有一定特征性，结合临床和实验室检查可提高诊断准确率。

Diagnostic value of CT in subacute thyroiditis

Objoctiw To investigate the diagnostic value of CT in subacute thyroiditis （SAT）, in order to increase the diagnositic ability. Methods The findings of non-enhanced and enhanced CT in 9 cases of SAT confirmed by surgery, pathological and laboratory examination were retrospectively analyzed with regard to the location, size, contour, density, margin, contrast enhanced degree, margin of the glandular organ and lymph node of neck. Rosults There were 5 cases of deffused type, in 4 cases among which isthmus is invloved, with illegible edge and low density, and marked ehhancement after enhanced scanning. There were 3 cases of local type. Solitary node was found by one side in one case, multiple-nodes were found by both sides in two cases, isthmus was invloved in one case. The nodes were 0.7-2.2cm in size, lower in desity than musculi skeleti, and unclear in edge, with mild or marked enhancement and unclear egde after enhanced scanning. There was one case in which local type and deffused type coexisted.with all hypothyroids swollen, clear edge, but no enlarged lymph nodes. Conclusion CT findings of SAT have characteristics. The accuracy of SAT diagnosis is to be increased if combined with clinic and laboratory examination.
