自拟逐瘀祛邪补虚调治方治疗慢性肝病肝纤维化的临床观察

目的探讨自拟逐瘀祛邪补虚调治方治疗慢性肝病肝纤维化的临床疗效。方法将172例慢性肝病肝纤维化患者随机分为治疗组92例和对照组80例。两组患者均给予常规综合治疗,治疗组加用逐瘀祛邪补虚调治方,对照组加用大黄虫丸,疗程为6个月。观察两组患者治疗前后临床症状、体征、肝功能、肝纤维化指标以及肝脏彩色多普勒变化情况。结果治疗组治疗后总有效率为82.4%,对照组总有效率为68.5%,两组比较,差异有统计学意义(P<0.01);两组治疗后肝功能、肝纤维化指标均明显改善,且治疗组明显优于对照组(P<0.05或P<0.01);治疗后肝脏彩色多普勒检查显示两组肝门静脉和脾静脉内径、脾肿大程度均较前明显减小,且两组比较,差异有统计学意义(P<0.05或P<0.01)。结论自拟逐瘀祛邪补虚调治方治疗慢性肝病肝纤维化具有明显疗效,值得临床推广和进一步研究。

Clinical Observation of Hepatic Fibrosis in Chronic Liver Disease Treated with the Prepared Formula for Removing Stasis, Eliminating Pathogens and Tonifying the Body

Objective To explore the clinical efficacy on hepatic fibrosis in chronic liver disease （CLD） treated with the prepared formula for removing stasis, eliminating pathogens and tonifying the body （ the prepared formula）. Methods One hundred and seventy - two cases of CLD hepatic fibrosis were ran- domized into a treatment group （ 92 cases ） and a control group （ 80 cases）. The conventional comprehensive therapy was applied in the two groups. In the treatment group, the prepared formula was supplemented. In the control group, dahuang shuchong wan was supplemented. The session of treatment was 6 months. The clinical symptoms, physical signs, liver function, indexes of hepatic fibrosis and color Doppler changes of liver were observed before and after treatment for the patients in the two groups. Results The total effective rate was 82.4% in the treatment group and was 68.5 % in the treatment group after treatment, presenting the statisti- cally significant difference in comparison（ P 〈 0.01 ）. The liver function and indexes of hepatic fibrosis were all improved apparently after treatment in the two groups and the results in the treatment group was apparently superior to the control group （ P 〈 0.05, P 〈 0.01 ）. The liver color Doppler after treatment all displayed that the inner diameters of hepatic portal vein and splenic vein were smaller and the splenic swelling was reduced apparently in the two groups as compared with those before treatment. And the differences were significant be- tween the two groups （ P 〈 0.05,P 〈 0.01 ）. Conclusion The prepared formula for removing stasis, elimina- ting pathogens and tonifying the body achieves the apparent efficacy on CLD hepatic fibrosis and deserves to be promoted in clinical practice and have a further study on it.
