重组人血管内皮抑素对脉络膜新生血管的抑制作用

摘    要:目的 探讨重组人血管内皮抑素对半导体激光诱导的Brown Norway大鼠脉络膜新生血管(choroidal neovascuIariza-tion,CNV)的抑制作用.方法 取雄性大鼠30只(60眼),大鼠双眼眼底采用半导体激光光凝建立CNV模型,随机分为3组:实验组、生理盐水组及空白对照组,每组10只(20眼).实验组玻璃体内注射重组人血管内皮抑素20 μL(5g·L-1),每2d注射1次,共注射5次;生理盐水组玻璃体内注射生理盐水20μL,每2d注射1次,共注射5次;空白对照组光凝后不做处理,观察期为14d.观察术后13d各组大鼠荧光素渗漏情况,术后14d光镜下观察组织细胞学变化,免疫组织化学检测CD105的表达,以及应用激光共聚焦显微镜下脉络膜血管平铺法测量各组CNV面积.结果 FFA显示实验组严重渗漏发生率明显低于生理盐水组和空白对照组;实验组光镜下光凝部位新生血管和内皮细胞数明显低于对照组;CD105的表达实验组明显低于生理盐水组和空白对照组;激光共聚焦显微镜下CNV定量分析显示实验组CNV面积(17352±1321)μm2小于生理盐水组(23043±1424)μm2和空白对照组(25937±1744)μm2,其差异具有显著统计学意义(F=342.4,P<0.001).结论 重组人血管内皮抑素(恩度)可以有效抑制动物模型的CNV形成,为治疗CNV提供了一种新的可能途径.

Abstract:Objective To investigate the inhibitive effect of recombinant human endostatin on the experimental choroidal neovascularization (CNV) model induced by semiconductor laser photocoagulation.Methods Experimental CNV model was induced by semiconductor laser in 30 male rats (60 eyes ) and randomly divided into experimental group,saline control group and normal control group,10 cases (20 eyes) in each group.The recombinant human endostatin (5 g·L-1) of 20 μL was intravitreously injected in experimental group,1 time in each 2 days with a total of 5 times.Saline of 20 μL were intravitreously injected in saline control group with,1 time in each 2 days with a total of 5 times.Normal control group were given no any treatment after photocoagulation.At 13 days after photocoagulation,the fluorescein leakage was observed in each group by fundus fluorescein angiography.At 14 days after photocoagulation,the pathology and ultrastructure were observed by light microscopy,the expression of CD105 were measured by immunohistochemistry,CNV area were measured by choroidal flatmount method under laser confocal microscope.Results The grave leakage rate of CNV in experimental group was obviously lower than those in saline group and normal control group.The number of new vessels and endothelial cells around the photocoagulation area in experimental group were obviously lower than those in normal control group.CD105 expression in experimental group was significantly lower than those in saline group and normal control group.Under the light microscope,CNV area in experimental group (17 352±1321)μm2 was significantly smaller than those in saline control group (23 043±1424)μm2 and normal control group (25 937±1744)μm2,there was significant difference(F=342.4,P<0.001).Conclusion The recombinant human endostatin can effectively inhibit CNV in rat models,which may be a useful new method in treatment of CNV.
