妊娠相关血清蛋白A检测及临床意义

目的:探讨妊娠相关血清蛋白A(PAPP-A)作为血清标记物在产前筛查中的应用价值。方法:用酶联免疫吸附法(ELISA)测定孕9周～20周孕妇血清中PAPP-A浓度,检测结果低于正常下限值的孕妇视为不良妊娠的高危人群,并对其进一步行羊水染色体检查及B超检查,每例受检的孕妇随访追踪到胎儿出生。结果:流产、死胎及胎儿宫内发育迟缓、畸形、染色体异常、白化病等的孕妇血清PAPP-A浓度降低。孕期PAPP-A浓度异常的孕妇易发生羊水过多或过少、胎儿宫内窘迫、羊水早破、妊高症等并发症。结论:孕妇血清PAPP-A检测在产前筛查中有重要意义,对于PAPP-A低值的孕妇提示有不良妊娠结局的可能。

Clinical Significance of Determination of Serum Pregnancy Associated Plasma Protein-A

Objective To Investigate the evalue of determination of the serum marker pregnancy associated plasma protein(PAPP-A)for screening pregnancy.Methods Serum PAPP-A concentration were determined with ELISA in early(9 weeks～20 weeks)Pregnancy.Women were assumed in high risk group of unfavorable outcome of the Pregnancy if their PAPP-A concentration were below lower limit.Chromosome examination of amnioticfluid and B-ultrasonography were further performed,follow-up study was continued until Fetalbirth.Results Serum PAPP-A concentration were depressed in Abortion,Stillbirth,Hypoplasia,Deformity,Chromosome abnormal and Albinism etc.Complication easy happened if Serum PAPP-A concentration was abnormal in pregnancy.such as amnioticfluid too more or little,amniorrhexis early and pregnancy-induced hypertersion syndrome etc.Conclusion PAPP-A is a very useful marker for Pregnancy screening,and depressed levels also suggest apossible unfavorable outcome of the Pregnancy.
