锁定钢板治疗老年股骨远端骨折34例临床分析

目的评估锁定钢板治疗老年股骨远端骨折的临床疗效。方法将2008年1月—2011年12月收治的68例老年股骨远端骨折患者采用解剖型钢板(对照组)和锁定钢板(试验组)内固定治疗,统计并比较两组患者手术时间、术中出血量、愈合时间、术后并发症发生率与膝关节功能恢复情况。结果锁定钢板组平均手术时间为(147±16)min,解剖型钢板组为(152±21)min,两组比较无统计学差异(P>0.05);锁定钢板组术中出血量为(340±14)ml,骨骼平均愈合时间为(118±24)d,解剖型钢板组术中出血量为(460±16)ml,骨骼平均愈合时间为(143±18)d,两组比较差异有统计学意义(P<0.05);两组术后并发症发生率比较,锁定钢板组为5.8%,解剖型钢板组为26.4%,差异有统计学意义(P<0.05);膝关节恢复功能比较,锁定钢板组优良率为85.3%,解剖型钢板组为50.0%,锁定钢板组功能恢复情况明显优于解剖型钢板组(P<0.05)。结论锁定钢板治疗老年股骨远端骨折操作简单,对骨折血供破坏小,能取得较好的固定效果,膝关节功能恢复良好,患者可进行早期锻炼,术后并发症少,具有良好临床效果。

Clinical Study of Locking Plate in Treatment of 34 Senile Cases with Distal Femoral Fractures

Objective To discuss the clinical effect of locking plate on senile distal femoral fractures.Methods We retrospectively studied 68 cases of senile distal femoral fractures admitted to our hospital and treated with locking plate(trial group) and anatomical plate(control group) from Jan 2008 to Dec 2011.The operative time,blood loss,healing time,incidence of postoperative complications and recovery of knee function were analyzed and compared between two groups.Results The trial group was better than the control group in all the following parameters.Operative time was(147 ± 16) vs.(152 ± 21) min,there was no significant difference(P > 0.05).Blood loss was(340 ± 14) vs.(460 ± 16) ml,and average bone healing time was(118 ± 24) vs.(143 ± 18) days,the difference between two groups has statistical significance(P < 0.05).Incidence of postoperative complications was 5.8% vs.26.4%,the difference has statistical significance(P < 0.05).Good and excellent rate of knee function recovery was 85.3% vs.50.0%(P < 0.05).Conclusion The application of locking plate to the treatment of senile distal femoral fractures is of simple operation,stable fixation and good recovery of knee joint function.
