大鼠被动型海曼肾炎肾脏p21及增殖细胞核抗原的表达及意义

摘    要:目的研究大鼠被动型海曼肾炎(passive heymannn ephritis,PHN)肾脏p21及增殖细胞核抗原(proliferating cell nuclear antigen.PCNA)的表达及其与PHN发病的关系。方法32只SD大鼠随机分为PHN模型组及正常对照组,定期检测24h尿蛋白定量及血液生化指标,同时取肾皮质标本切片行光镜、免疫荧光及电镜检查,采用S越3C法分别检测肾脏p21及PCNA的表达。结果PHN模型组大鼠出现大量蛋白尿及高胆同醇血症,肾小球内可见免疫复合物沿毛细血管壁沉积,电镜下见。肾小球上皮下电子致密物沉积。免疫组织化学显示,PCNA及p21在PHN组肾小球内的表达均较对照组明显增高(P〈0.05)。结论PHN模型中肾小球足细胞显示一定的低增生能力,可能部分由于补体损伤后p21的水平上调所致,它对促进PHN后期病程进展及膜性肾病的预后具有意义。

Abstract:Objective To investigate the expression of p21 and PCNA in rat passive heymann nephritis(PHN) and its correlation with pathogenesis of PHN. Methods Thirty-two male SD rats were randomly divided into two groups:control group (n = 16) and PHN model group (n = 16) . The rats in PHN model group were induced by antigen FX1A peritoneal injection according to Edgington's methods. The 24 h urinary protein was measured every week and the serum total protein,albumin, total cholesterol and creatinine were determined regularly. The rats were sacrificed, then the renal cortex was processed for light microscopy, direct immunofluorescence and electron microscopy, and the renal PCNA and p21 expression was detected by SABC method. Results Compared with control group,the rats in PHN model group showed massive proteinuria and increased serum total cholesterol (P〈 0. 05). The examination of immunofluorescence showed that strong positive immunofluorescence deposited along the capillary wall in glomeruli of PHN model group, and electron microscopy revealed that the glomerular subepithelial electron dense deposition appeared in PHN model group. Weak expression of PCNA appeared in glomeruli of control group, and no expression of p21 was detected in control group rats. The expression of PCNA and p21 was both increased significantly in glomeruli of PHN model group (P〈0. 05). Conclusions The podocytes in PHN rats show some low proliferative capacity, which is likely, in part, caused by the up-regulation of p21 expression, and this state may participate in the pathogenesis of PHN and promote the progress of PHN.
