宫颈上皮病变中高危型HPV感染与病变分级及P53、p16INK4a和细胞周期蛋白表达的关系

摘    要:目的研究宫颈上皮病变(CIN)中高危型人乳头瘤病毒(HR-HPV)感染与病变分级及P53、p16INK4a和细胞周期蛋白表达的关系。方法回顾性收集首都医科大学附属北京妇产医院2014年3月至2018年3月期间接受宫颈活检、宫颈高频电刀圈环切除、宫颈冷刀锥切的CIN组织。测定HR-HPV的感染类型,并比较HR-HPV感染与否、不同病变分级CIN患者中P53、p16INK4a、细胞周期蛋白的蛋白表达水平。结果 84例CIN患者中,HR-HPV感染率为30. 95%;其中HPV16、HPV18为主要的HR-HPV感染类型,感染率分别为11. 90%、10. 71%。不同病变分级CIN中HR-HPV感染率以及P53、p16INK4a、Cyclin D1、Cyclin B1的蛋白阳性表达率的比较有显著性差异(P <0. 05);随着CIN分级的升高,HR-HPV的感染率以及Cyclin D1、Cyclin B1的蛋白阳性表达率明显升高,P53、p16INK4a的蛋白阳性表达率明显降低;合并HR-HPV感染的CIN中P53、p16INK4a的蛋白阳性表达率明显低于不合并HR-HPV感染的CIN,Cyclin D1、Cyclin B1的蛋白阳性表达率明显高于不合并HR-HPV感染的CIN(P <0. 05)。结论 CIN中HR-HPV感染与病变分级加重及P53、p16INK4a、细胞周期蛋白表达改变有关。

Abstract:Objective To study the relationship between high-risk human papillomavirus( HR-HPV) infection in cervical intraepithelial neoplasia( CIN) and pathological grade,P53,p16 INK4 a and cyclin expression. Methods CIN tissues were collected from our hospital from March 2014 to March 2018,who underwent cervical biopsy,cervical high frequency electric knife ring excision and cervical cold knife conization. The infection of HR-HPV and the expression levels of P53,p16 INK4 a and cyclin were determined. Results HR-HPV infection rate was 30. 95% in 84 patients with CIN,and HPV16 and HPV18 were the main types of HR-HPV infection,the infection rate was 11. 90% and10. 71% respectively. The infection rate of HR-HPV and the positive expression rate of P53,p16 INK4 a,CyclinD1 and CyclinB1 in CIN with different pathological grade were significantly different( P < 0. 05). With the increase of CIN grade,the infection rate of HR-HPV and the positive expression rate of CyclinD1 and CyclinB1 were significantly increased,and the positive expression rates of P53 and p16 INK4 a were significantly decreased. The positive expression rates of P53 and p16 INK4 a in CIN with HR-HPV infection were significantly lower than those without HR-HPV infection,while the positive expression rates of CyclinD1 and CyclinB1 were significantly higher than those without HR-HPV infection.Conclusion HR-HPV infection in CIN is related to the aggravation of lesion grade and the changes of P53,p16 INK4 a and cyclin expression.
