中老年高血压患者轻度认知损害的中医证候及神经心理研究

目的探讨中老年高血压伴轻度认知损害(MCI)患者的中医证候特点及其神经心理学特征,为中医早期干预,防治血管性痴呆提供辨证依据。方法选择有轻度MCI的中老年高血压患者200例,在应用Folstein简易智能状态检查表(MMSE)和临床记忆量表对患者进行认知评估后,参照《血管性痴呆辨证量表》拟订MCI的辨证草案,对患者进行辨证,并对证候进行定量积分。结果MCI患者的最常见证候是肾精亏虚证,其次是痰浊阻窍证、瘀血阻络证、肝阳上亢证、热毒内盛证。结论中老年高血压患者最常见的证候与认知功能损害之间具有密切关系,从这些因素入手,对中老年高血压患者防治MCI及血管性痴呆具有重要意义。

Syndromic and neuro-psychological studies of the middle and aged patients suffering from hypertension with mild cognitive impairment

Objective It is to explore the characteristics of syndrome and neuro-psychology of the middle and aged patients suffering from hypertension with mild cognitive impairment(MCI),to provide dialectical evidence for early intervention and prevention with TCM against dementia.Methods The middle and aged patients suffering from hypertension with MCI were chosen as observing objects.Folstein's mini-mental state examination(MMSE) and clinical memory scale were applied to evaluating their cognition.Then a syndrome differentiation plan of MCI was made via the reference to differentiation scale of vascular dementia with which the patients' syndromes were differentiated and their quantitative integrals were defined.Results The most common syndrome of the MCI patients in this group was kidney deficiency.The other ones were phlegm obstructing orifices,blood stasis in collaterals,hyperactivity of liver-Yang and heat-poison in the interior.Conclusion There is a relationship between the syndromes and cognitive impairment with the unique characteristics in the field of neuro-psychology.It is significantly important for the patients mentioned above to prevent and treat for vascular dementia according to these factors.
