海澡酸钠微球栓塞治疗肝血管瘤

目的评价超选性肝动脉插管及海澡酸钠微球（KMG）栓塞对肝血管瘤的临床疗效及安全性。方法 53例肝血管瘤男31例,女22例,年龄（25岁～56岁,平均年龄41.8岁）均通过超声、CT、MRI、DSA发现病灶,36例经术前影像学确诊,17例经DSA确诊,53例在术中均行DSA。确诊后行海澡酸钠微球11ml～30ml栓塞治疗。观察术后疗效、症状及并发症情况。结果 53例患者均经DSA明确诊断,17例术前未确诊者行DSA后,可见＂早出晚归＂、＂挂果样＂、＂爆米花样＂、或＂棉花团状＂改变等特征性征象。53例于栓塞后1个月～12个月超声、CT或MRI随诊,瘤体缩小〉50%者37例,〉30%者14例,〉10%者2例,有效率100%,术中、术后无严重并发症。结论超选性肝动脉插管及KMG栓塞对肝血管瘤的治疗是安全、有效的治疗方法。

Seaweed sodium microspheres embolization for hepatic hemangioma

Objective To evaluate the clinical efficacy and safety of super selected hepatic artery embolization with sea-bathing sodium microspheres（KMG） for liver hemangiomas.Methods Fifty-three cases with hepatic hemangioma,31 males and 22 females,age（25 to 56 years old,mean age 41.8 years old） were adopted by ultrasound,CT,MRI,DSA.Thirty-six patients were diagnosed by preoperative imaging,17 cases diagnosed by DSA,and all the 53 cases received DSA.Embolization was done with seaweed 11ml-30ml of sodium microspheres after definite diagnosis.The curative effect,symptoms and complications were observed.Results Fifty-three patients were diagnosed by DSA,and in 17 patients suspected hepatic hemangioma,DSA showed the visible ＂morning to evening,＂ ＂fruiting-like＂,＂popcorn-like＂,or ＂cotton-like groups,＂ and changes in the characteristic signs.At 1-12 months after embolization,ultrasound,CT or MRI followed up were done.Tumor shrinked 50% in 37 cases,30% in 14 cases and 10% in two cases.The efficiency was 100%.No serious complications occurreed.Conclusion Super selected KMG hepatic artery embolization for the treatment of liver hemangioma is safe and effective.
