摘    要:目的 探讨社区2型糖尿病患者血尿酸水平与代谢综合征的关系。方法 社区1008例2型糖尿病患者纳入本研究,根据血尿酸三分位水平将患者分为3组:T1(n=367)、T2(n=321)和T3(n=320),比较3组患者的临床特征,分析血尿酸水平与代谢性综合征的关系。结果 3组患者在糖尿病病程、高血压、体重指数、腰臀比、肌酐、总胆固醇、高密度脂蛋白-胆固醇、空腹血糖、餐后2h血糖、空腹C肽、餐后2h C肽、糖基化血红蛋白间的差异有统计学意义(P<0.05);3组患者代谢综合征发生率依次升高,分别为34.1%、46.1%和64.7%(P=0.000);在校正年龄、性别、糖尿病病程、吸烟史和饮酒史等混杂因素后,以低位组作为参考,中位组和高位组患者代谢综合征的危险度分别增加1.580倍(95% CI:1.154~2.164,P=0.000)和3.611倍(95% CI:2.539~5.315,P=0.000);2型糖尿病患者中血尿酸较高者更易合并4~5个代谢综合征组分;Binary Logistic回归分析结果显示,血尿酸三分位水平与代谢综合征的发生率显著相关(P<0.05)。结论 社区2型糖尿病患者血尿酸水平与代谢综合征密切相关,血尿酸水平增高是2型糖尿病患者代谢综合征发生的一个独立危险因素。

Abstract:Objective To investigate the relationship between serum uric acid(SUA) and metabolic syndrome(MS) in type 2 diabetes. Methods Totally 1008 type 2 diabetes in patients were included and were divided into three groups, T1 (n=367),T2(n=321) and T3 (n=230) according to the SUA level. The epidemiologic feature and the prevalence of MS was compared among three groups. Results There were significant differences in diabetic duration, hypertension, BMI, WHR, CR, TG, HDL-TG, FBG, 2h FBG, C-peptide, 2h C-peptide,GHb among three groups. The prevalence of MS among three groups were 34.1%,46.1% and 64.7% respectively, which mounted up gradually but significantly. Compared with the lower group, the odd ratios of MS in the other two group were respectively 1.580(95% CI:1.154-2.164,P=0.000)and 3.611(95% CI:2.539-5.315, P=0.000). People with diabetes in the higher SUA tertiles were more likely to exhibit four or five metabolic syndrome components.Binary Logistic regression analysis showed there was significant association between SUA tertiles and the presence of metabolic syndrome in type 2 diabetic inpatients (P<0.05). Conclusion There was intimate association between Serum uric acid and metabolic syndrome in type 2 diabetes. SUA is a independent risk factor of metabolic syndrome.
