摘    要:目的探讨呼吸机相关肺炎(VAP)的病原菌以及药物敏感情况和初始经验性抗生素的治疗策略。方法对2004年3月~2004年9月收治的38例VAP患者的病原菌分布、药敏情况进行回顾性分析。结果VAP感染的致病菌中,革兰阴性(G-)菌为主,占75.9%,革兰阳性菌(G )占24.1%,其中铜绿假单胞菌占44.8%,金黄色葡萄球菌占24.1%,混合感染占57.9%,真菌感染较常见。结论VAP感染的致病菌以G-菌为主,混合感染以及耐药率有所增加,临床上必须注意病原菌的分布和耐药情况变化,合理选择抗生素。

Abstract:Objective To analyze the pathogens,drug-sensitive conditions and initial empirical therapeutic tactics with antibiotics of ventilat or-associated pneumonia(VAP).Methods A review study was carried out concerning the distribution of pathogens and drug sensitive conditions in 38 cases of VAP admitted from March to September 2004.Results G~-bacteria,predominating among the pathogens in the infection of VAP,were 75.9 percent;and G~ bacteria were 24.1 percent,including 44.8 percent of Pseudomonas aeruginosa,24.1 percent of Staphylococcus aureus,and 57.9 percent of mixed inf ections.Besides,the infections of Fungi were quite common.Conclusion G~-bacteria predominate among the pathogens in the infection of VAP,and mixed infections and drug-resistant rates have increased slightly.Vigilance has to be placed to the distribution of the pathogens and the variation of drug-resistant conditions on which to base antibiotics choice.
