摘    要:目的:探讨经腹超声测量膀胱质量(ultrasound-estimated bladder weight,UEBW)对膀胱出口梗阻(bladder outlet ob-struction,BOO)的临床意义及尿动力学指数的相关性。方法:将有下尿路症状的70例患者分为膀胱出口梗阻组、非膀胱出口梗阻组和对照组,比较组间的UEBW与年龄、最大尿流率、残余尿量、国际前列腺症状评分的差异。结果:梗阻组与非梗阻组、对照组UEBW比较,P<0.001,有显著性意义。单回归曲线中UEBW与残余尿量有显著相关性(r=0.49;P<0.001)。它对BOO的诊断准确率为87.1%(61/70),假阳性率和假阴性率分别为11.4%(5/44)、14.3%(3/21)。结论:UEBW可作为一种非侵袭性预测BOO的指标,有广泛的临床应用价值。

Abstract:Objective:To predict the clinical utility for bladder outlet obstruction (BOO) using ultrasound-estimated bladder weight (UEBW) and to reveal the correlation of UEBW with urodynamics tests. Methods:A total of 70 men with lower urinary symptoms were divided into BOO group, non-BOO group and control group. UEBW of the subgroups was compared with age, maximum flow rate, postvoid residual urine, and international prostatic symptoms scores (IPSS). Results:UEBW of Boo greup was statistical significance compared to non-Boo group and controegroup (P<0.001). In a simple regression analysis, there was a statistically significant correlation between UEBW and postvoid residual urine (r=0.49;P<0.001). It revealed a diagnostic accuracy of 87.1% (61/70) for bladder outlet obstruction with 11.4%(5/44) and 14.3% (3/21) false-positive and false-negative rates, respectively. Conclusion:UEBW may be used to predict BOO noninvasively and it has an extensive clinical utility.
