摘    要:目的探讨震荡排痰器对老年人的肺功能的影响。方法60例85岁或以上的老年人随机分为干预组和对照组。对两组均进行简易肺功能测定,检测指标包括峰流速(peak expiratoryflow,PEF)、1s用力呼气容积(ferced expiratory volume in the firstsecond)、用力肺活量(forced vital capacity,FVC)、1S用力呼气容积与用力肺活量的比值(FEV1/FVC%)。干预组予以震荡排痰器进行肺康复锻炼,对照组不作任何干预。28d后比较干预组与对照组的肺功能、发热天数、抗生素使用天数及外出就医例数。结果两组肺功能方面,干预前PEF、FEV1、FVC、FEV1/FVC%两组问均无显著性差异(P〉0.05),干预后,PEF、FEV1、FEV1/FVC%干预组、对照组间无显著性差异(P〉0.05);干预组较对照组用力肺活量有增加,FVC两组间有显著性差异(P〈0.05),前后的差值分别为(0.33±0.30)L和(0.20±0.14)L。发热例数、抗生素使用例数和外出就医例数方面,干预组和对照组无显著性差异(P〉0.05)。结论高龄老年人肺功能减退,采用物理排痰器进行肺康复锻炼可以改善肺功能,改善肺活量。

Abstract:Objective To explore the effects of flutter valve mucus clearance devices on pulmonary function in senior patients. Methods Sixty senior patients aged 85-years or above were included and allocated randomly into the intervention group and control group. Pulmonary function tests were performed in patients of both groups, including indicators like peak expiratory flow ( PEF), in the first second forced expiratory volume (FEV1), forced vital capacity (FVC), and ratio of FEV1 to FVC (FEV1/FVC%). Patients in the intervention group were given flutter valve mucus clearance devices to execute pulmonary function exercise, while patients in the control group received no intervention. After 28 days, the pulmonary function, number of cases with fever, antibiotic therapy and hospital visits were compared between the two groups. Results The pulmonary functional indicators PEF, FEV1, FVC, and FEV1/FVC% showed no significant difference between the two groups before intervention, and both were at baseline ( P 〉 0.05 ). After intervention, three indicators, PEF, FEV1, and FEV1/FVC%, showed no significant difference between the two groups (P 〉0.05 ). However, FVC altered before and after 28 days in the patients of the intervention group, showing (0.33 ±0.30) L and (0.20 ± 0.14) L in the intervention group and control group, respectively, and the difference was statistically significant ( P 〈 0.05 ). There is no significant difference in the number of cases with fever, antibiotics therapy and hospital visits between two groups ( P 〉 0.05 ). Conclusion Pulmonary function in senior patients is generally deteriorating, and the application of flutter valve mucus clearance devices in these people for pulmonary function exercise can improve the pulmonary function by increasing forced vital capacity.
