| 摘 要： | 目的探讨微创手术在诊断和治疗肺良性肿瘤中的应用价值.方法 2007年7月至2010年12月共收治肺良性肿瘤患者73例,69次（同期双侧3次）微创手术,其中包括VATS（video-assisted thoracoscopicsurgery）电视胸腔镜手术和VAMT（video-assisted mini-thoracotomy）电视胸腔镜辅助小切口手术,切除瘤体72个.手术方式：肺楔形切除术22例（其中3例双侧VATS）,肺叶切除术11例,肺肿瘤摘除术36例,中转开胸4例.VATS为2-3个小切口;VAMT为辅助5～8cm左右小切口.结果全组无围手术期死亡病例,术中出血量（VATS：50～130 mL）/（VAMT：110～200 mL）;手术时间（VATS：110 min/VAMT：125 min）;住院时间（VATS：4～5 d）/（VAMT：5～7 d）;术口平均长度为5～10 cm.术后病理：结核球10例,炎性假瘤18例,慢性肉芽肿11例,肺囊肿6例,错构瘤21例,硬化性血管瘤3例.结论微创手术在肺良性肿瘤是一种值得推广的诊断和治疗方法,有广泛的开展价值. |
