联合检测血清肿瘤标记物在恶性肿瘤诊断中的价值

摘要:目的　探讨联合检测血清肿瘤标记物在恶性肿瘤诊断中的价值。方法　选择2012年3月至2013年3月某院体检中心进行健康体检及门诊就诊患者共14280例。根据恶性肿瘤筛查表结果、患者临床症状及阳性体征综合判断，最终确定恶性肿瘤高危人群3587人，对其血清肿瘤标记物CA125、AFP、CEA、CA19-9、CA72-4、NSE进行检测。对血清肿瘤标记物异常升高患者进行全面检查和长期随访（12个月）。结果　随访1年发现恶性肿瘤患者59例，通过Cox回归多因素分析发现，CA125、CEA、CA19-9、CA72-4、NSE是早期筛查恶性肿瘤的危险因素（P＜0.05），其中CEA（RR:8.325）、CA19-9（RR:6.761）、CA125（RR:2.231）是重要的危险因素。结论　联合检测血清肿瘤标记物有助于癌症的早期诊断、治疗，而且也是恶性肿瘤的危险因素。

The Diagnosis Value of Combination Detection of Different Serum Tumor Markers for Malignant Tumor

Abstract: Objective This work was to investigate the diagnosis value of the combination detection of different serum tumor markers for malignant tumor. Methods From March 2012 to March 2013, a total of 14280 subjects from the Medical Examination Center and Clinics of our hospital were studied, and 3587 subjects were divided into high-risk groups according to the tumor screening results and clinical symptoms. The levels of tumor markers (CA125, AFP, CEA, CA19-9, CA72-4, NSE) in high-risk tumor groups were measured, and the patients with abnormal results were followed up for 12 months. Results 59 cases were diagnosed as malignant tumor after 1-year follow-up. The result of Cox regression analysis showed that CA125, CEA, CA19-9, CA72-4, NSE were risk factors for early malignant tumor (P<0.05). CEA (RR: 8.325), CA19-9 (RR: 6.761), CA125 (RR: 2.231) were important risk factors. Conclusion The combination of different serum tumor markers for malignant tumor detection is helpful for the diagnosis and treatment of cancer.
